ThisnotificationismadepursuanttoCookNuclearPlant'sNPDESPermitMI0005827,PartII,A.5.OnNovember8,1997,duringcontinuouschlorinationoftheServiceWaterSystems,plantpersonnelinadvertentlydischargedaportionofchlorinatedServiceWatertoOutfall002forapproximatelyninehourswithoutperformingtherequiredsamplingandanalysisforTotalResidualOxidant.ThesesamplesweremissedduetoavalvelineuperrormadeduringthestartupofcontinuouschlorinationafterashortshutdownforgeneralmaintenanceontheCirculatingWaterSystem.Shortlyafterthelineupwascorrected,asamplewastakenfromtheOutfall002dischargesamplepointat10:15a.m.onNovember8,1997.Thesamplemeasured0.07ppmTotalResidualOxidant(TRO),whichexceededthe0.036ppmmaximumdischargelimit.TheconditionwasdiscoveredduringatourbyplantpersonnelatwhichpointthechlorinationsystemwasshutdownNovember8,1997at-09:00a.m.Shortlyafterthediscovery,thevalvewasrepositionedtopreventdischargefromOutfall002anddirectalltheServiceWatertotheOutfall001,wherecirculatingwaterdilutionispresent.ContinuouschlorinationresumedNovember8,1997at09:45a.m.AbriefingwasheldbytheplantpersonnelinvolvedwithchlorinationtoemphasizetheimportanceofcheckingtheCirculatingWaterSystemvalvelineuppriortocommencingchlorinationasrequiredintheirprocedure.ThechlorinationprocedureincludesstepstoensurethatpropervalvelineupshavebeenperformedtoavoidNPDESexceedences.
