SUBJECT: NRC INSPECTION REPORT NO. 50-382/99-05

Dear Mr. Dugger:

This refers to the inspection conducted on February 28 through April 10,1999, at the Waterford

Steam Electric Station, Unit 3, facility. The enclosed report presents the results of this

inspection.

During the 6-week period covered by this inspection, your conduct of activities at Waterford 3

was generally characterized by safety-conscious operations, sound engineering and

maintenance practices, and careful radiological controls. However, the number of personnel

errors, identified during Refueling Outage 9 demonstrated a lack of attention to detail by plant

personnel. The errors discussed in Sections O4.1, M4.1, and S1.1 of the enclosed report

involve a number of different onsite organizations. These findings indicate that further

management attention may be warranted to improve human performance and reduce the

number of personnel errors. l

Based on the results of this inspection, the NRC has determined that three violations of NRC

requirements occurred. These violations are being treated as noncited violations (NCV),

consistent with Appendix C of the Enforcement Policy. These NCVs are described in the

subject inspection report. If you contest the violation or severity level of these NCVs, you

ishould provide a response within 30 days of the date of this inspection report, with the basis for

.your denial, to the Nuclear Regulatory Commission, ATTN: Document Control Desk,

Washington, DC 20555-0001, with copies to the Regional Administrator, Region IV, and the

Director, Office of Enforcement, United States Nuclear Regulatory Commission, Washington,

DC 20555-0001.

In accordance with 10 CFR 2.790 of the NRC's " Rules of Practice," a copy of this letter, its

enclosure, and your response will be placed in the NRC Public Document Room (PDR).
