APPENDIX B Valley Steel Products Company Docket No. 99901019/35-01 NOTICE OF NONCONFORMANCE Based on the results of an NRC inspection conducted on November 20, 1985 it appears that certain of your activities were not conducted in accordance with NRC requirements.
Criterion V of Appendix B to 10 CFR Part 50 states: " Activities affecting quality shall be prescribed by documented instructions, procedures, or drawings, of a type appropriate to the circumstances and shall be accomplished in accordance with these instructions, procedures, or drawings. Instructions, procedures, or drawings shall include appropriate quantitative or qualitative acceptance criteria for determining that important activities have been satisfactorily accomplished."
Section 11.2 of the Valley Steel Products Company Quality Assurance Identifi-cation and Verification Program dated April 1, 1981 states, in part, "... vendor qualification activity will consist of performing initial surveys and periodic audits of the vendor's quality systems program...."
Section 11.3 states, in part, " Based en results of the above [ vendor surveys],
a determination of vendor acceptability is made...and the vendor is added to the Qualified Vendor List...."
Contrary to the above, a review of calibration records and vendor audits from 1978 through 1983 indicated that Weber Gage, Radiatronics, and Ehrhardt Tool and Machine Company (ETMC) had performed calibration services for Valley Steel, but Weber Gage and Radiatronics were not on the Qualified Vendor List (QVL).
Although ETMC appeared on the QVL, there was no documented evidence that Valley Steel performed surveys or audits of any of the three vendors (85-01-02).
