The Toledo Edison Company is submitting, pursuant to 10CFR50.71, one (1) original plus ten copies of Revison 6 to the Updated Safety Analysis Report (USAR) for the Davis-Besse Nuclear Power Station, Unit No. 1. The annual submittal vill be issued in July, therefore this submittal does not constitute the annual update.

USAR Revision 6 reflects organization changes implemented between January 22, 1988 and April 22, 1988. This USAR change is being submitted in preparation for implementing the removal of the D-B organization from the Technical Specifications in accordance with Generic Letter 88-06.

Information contained in Revision 6 to the USAR applies only to organization changes in Chapter 13.1 and Chapter 17.2. Please insert the Revision 6 material, dated April, 1988, into the Updated Safety Analysis Report as per the attached Revison 6 Instruction Sheet.

D-B SECTION 13 13.0 CONDUCT OF OPERATIONS 13.1 ORGANIZATIONAL STRUCTURE Centerior Energy Corporation (Centerior) owns the Davis-Besse Nuclear Power Station and Cleveland Electric Illuminating Company. The Davis-Besse Nuclear Power Station 6 is owned as tenants in common by Toledo Edison Company and Cleveland Electric Illuminating Company. Toledo Edison is responsible for the design, construction and operation of the station.

13.1.1 Corporate Organization The overall nuclear related organization chart is shown on Figure 13.1-1. l ,

The organization operates under the Chairman and Chief Executive Officer who 6  ;

is responsible for directing all the affairs of the Company. 5 Reporting-to the Chief Executive Officer is the Executive Vice President who is responsible for directing the operation of Toledo Edison company. 5 6 The major responsibility areas of the company are identified as groups and are administered and headed by Senior Vice Presidents or Vice Presidents.

l5 l4 13.1.1.1 Engineering and Operations Group l5 The functions of the operations Mission in Toledo Edison are to manage effectively all activities which interface directly with customers; to ensure that individual customer requirements are expeditously handled; to plan for, design, and construct (or oversee the construction of) all necessary facilities for transmission and distribution of electrical energy directly to customers; e to act as an energy broker in buying and selling arrangements; and to operate and maintain all generating facilities, other than nuclear, and to dispatch all generating facilities for which the Company is responsible so as to ensure a continuous energy supply for the Company system and interconnection with other utilities.

SR. VICE PRESIDENT, ENGINEERING AND OPERATIONS. Responsible for the activities 5 of the Energy Supply and Customer Services engineering operations of the company.

D-B ASSISTANT VICE PRESIDENT, NUCLEAR. As Staff Director is responsible for insuring the execution by the staff of the policies, and directives received from the Vice President, Nuclear. Responsible for coordinating the supporting -

administration, perconnel and security services to the Nuclear Group.

Directly supervises the Nuclear Health and Safety, and Information Management 5 Divisions and the Personnel and Special Programs Departments.

13.1.1.3 Finance and Administration Group l5 The function of the Finance and Administration Group in Toledo Edison is to 5 l-evaluate, provide, and manage various financial administrative services which support overall Company operations.

VICE PRESIDENT, FINANCE AND ADMINISTRATION. Responsible for the Admini- 5 strative Services, Personnel, Treasury and Accounting activities of the company.

DAVIS-BESSE SERVICES DIRECTOR. Responsible for the Administrative Services l6 activities of the Company including Industrial Security, Procurement, and Administrative Services.

13.1.2 Management and Station Support Organization l The Management Organization is a composite of the Corporate Organization and the Station organization. It designates arer.s of responsibility, authority and coordination.

13.1.2.1 Industrial Security Division INDUSTRIAL SECURITY DIRECTOR. Directs, manages, and administers the corporate security program, with the exception of the Edison Plaza program to all Company a personnel facilities, properties, inventories and other assets including the protection of the general public which might be endangered as a result of security incidents. Reports to the Davis-Besse Services Director. l6 l-NUCLEAR SECURITY MANAGERS. Manage and supervise all nuclear security 15 activities, including the planning, designing, organizing, coordinating, and implementing of the Company nuclear security plan and programs. Report to 15 the Industrial security Director.

NUCLEAR SECURITY SUPERVISORS. Supervise and participate in security activities 5 l+

of the Company and assists in Company nuclear security programs. Supervise and direct the activities of the security officers at the Davis-Besse Station l l-as directed by the Nuclear Security Managers.

13.1.2.2 Quality Assurance Division QUALITY ASSURANCE DIRECTOR. Coordinates, develops and directs all quality assurance / quality control activities required by the government and Company to ensure adequate quality assurance and quality control coverage throughout design, procurement, construction, operation and maintenance of nuclear-fueled facilities. Directs code inspection activities of new and existing energy generation and utilization facilities. Reports to the Vice President, l5 l' Nuclear.

QUALITY SYSTEMS MANAGER. Develops and maintains Nuclear Quality Assurance 5 Program, trending and analyzing documented deficiencies, reviewing site procedures, and providing Quality Assurance training for Nuclear Group personnel.

D-B 13.1.3.1 Davis-Besse Staff DAVIS-BESSE PLANT MANAGER. Directs administrative, operation, technical,

. planning, security and maintenance activities to ensure conformance and a compliance with Federal and State Regulations and License require ints and in the continuous, safe, and efficient operation of the Davis-Besse Station.

INDUSTRIAL SAFETY COORDINATOR. Coordinates and perfonas activities to eliminate, reduce or limit hazardous and unsafe working conditions, provide maximum safety to employees and encourage active interest in accident prevention.

4 ADMINISTRATIVE COORDINATOR. Assists the Plant Manager in performing the duties l5 and functions required of his position. Responsible for performing special projects, review and draf ting of communications, serving on cemmittees, and making recommen-dations for management action as required.

13.1.3.2 Davis-Besse Operations 13.1.3.2.1 Operations Section The operations Section Organization is shown on Figure 13.1-2. l6 ASSISTANT PLANT MANAGER, OPERATIONS. Directs the operational activities of the Station. Provides safe and efficient operation of the plant by highly trained and motivated personnel in accordance with tne provisions of the plant operating license, technical specifications, Company and Group policies. l5 OPERATIONS SUPERINTENDENT. Supervise activities of day-to-day operations of the <

plant to ensure compliances with Station procedures and regulatory requirements. l Coordinates activities with other departments and between operating shifts.

OPERATIONS SUPERVISOR. Assist the Operations Superintendent in the day-to-day supervision of plant operations to ensure compliance with station procedures and regulatory requirements. Coordinates Operations Department activities with other department and operating shifts.

OPERATIONS ENGINEERING SUPERVISOR. Directs and coordinates the activities of the 5 Operations Engineering Section. Operations Engineering reviews plant operations and procedures, troubleshoots operational problems, and performs other duties in suppor t of plant operations.

D-B Operations Shift Organization SHIFT TEC}0ICAL ADVISOR SUPERVISOR. Directs and coordinates the activities of the STA's to ensure all responsibilities are carried out and assignments are completed on tirae. Controls the STA training program administered to the Assistant Shift Technical Advisors.  :

SHIFT TECHNICAL ADVISOR. Serves as an on-shift technical advisor to the Shift Supervisor, providing additional technical and analytical expertise in the control Room to support the diagnosis, mitigation, and termination of off-normal events. Evaluates &n-to-day plant operations for safety implications. ,

Performs Operating Experience Assessment reviews of industry and regulatory reports for applicability to Davis-Besse.

SHIFT SUPERVISOR. Directly supervises shift operations. Evaluates and t approves maintenance, testing power and mode changes to ensure efficient, continuous operation of the generation facility in such a manner so as to assure the health, welfare and safety of the general public.

ASSISTANT SHIFT SUPERVI3OR. Directs, coordinates, and participates in the operation of the nuclear reactar, turbines, plant auxiliaries, and accessories, while ensuring the health and safety of the public.

REACTOR OPERATOR. Qualified for and will maintain a Reactor Operatot-'s License. He operates the Nuclear Steam Supply System, Turbine Generator, their auxiliaries, and all other equipment to maintain continuous production with maximum safety and efficiency.

EQUIPMENT OPERATOR. Tends the Nuclear Steam Supply System, Turbine Generator, station auxiliaries, and associated equipment.

Each shift for the single unit at the facility will consist of:

9 D-B 17.2.1 ORGANIZATION The Toledo Edison organizations and the key positions within these organ-izations that are involved with the implementation of the Nuclear Quality Assurance Program are illustrated in Figure 17.2-1 Davis-Besse Nuclear

- Power Station Organizational Chart. The corporate support organizations such as Purchasing and Industrial Security are also illustrated on this chart. The positions reporting to the Vice President, Nuclear, are known as the "Nuclear Group". The positions reporting to the Finance and Admin-istration Vice President are known as the "Administrative Group".

A Nuclear Group Policy and Organization Manual establishes the policy and organization for the Nuclear Group positions. The manual also provides detailed position descriptions including position title, function, duties, and qualification requirements for each of the Nuclear Group positions shown in Figure 17.2-1.

, Toledo Edison may delegate the responsibility to other organizations for implementing certain Quality Assurance Program requirements. Although 5 delegated, the overall responsibility is always maintained by Toledo Edison. The monitoring of these organizations is described in Sections 17.2.4 and 17.2.18.

CHAIRMAN AND CHIEF OPERATING OFFICER The Chairman and Chief Executive Officer is responsible for directing all affairs of the company.

EXECUTIVE VICE PRESIDENT The Executive Vice President reports directly to the Chairman and Chief Executive Officer and is responsible for the overall safe and efficient operation of the Davis-Besse Nuclear Power Station.

FINANCE AND ADMINISTRATION VICE PRESIDENT The Finance and Administration Vice President reports directly to the Executive Vice President. He is responsible for administrative services which include purchasing and security activities for the Davis-Besse site. 6 i DAVIS-BESSE SERVICES DIRECTOR The Davis-Besse Services Director reports to the Finance and Administration Vice President and is responsible for purchasing, security, and facility services.

PROCUREMENT DIRECTOR The Procurement Director reports to the Davis-Besse Services Director and is responsible for purchasing and contracting of materials, equipment and services for Toledo Edison.

D-B INDUSTRIAL SECURITY DIRECTOR 6

The Industrial Security Director reports to the Davis-Besse Services Director and is responsible for the corporate security program for all Toledo Edison personnel, facilities, properties, inventories and other assets including the protection of the general public which might be endangered as a result of Toledo Edison security incidents. He is further responsible for ensuring that nuclear security activities at the Davis-Besse site are properly administered by the appointed Station Nuclear Security Managers in accordance with the approved Security Plan.

VICE PRESIDENT, NUCLEAR The Vice President, Nuclear reports directly to the Executive Vice President and is responsible for all matters relating to the engineering, 6 design, operations, maintenance and modification of the Davis-Besse Nuclear Power Station. He reserves the authority to conduct or order the auditing of any activity at any time to determine compliance to the requirements specified in the Nuclear Quality Assurance Manual. He is also responsible for conducting a periodic review of the overall 16 effectiveness of the Quality Assurance Program.

ASSISTANT VICE PRESIDENT, NUCLEAR S

The Assistant Vice President, Nuclear reports to the Vice President, Nuclear, and is responsible for ensuring that the staff executes the policies and directives received from the Vice President, Nuclear. This responsibility includes support services such as information management, special administrative tasks and nuclear health and safety which includes emergency preparedness, environmental compliance and industrial safety and hygiene.

INFORMATION MANAGEMENT DIRECTOR The Information Management Director reports to the Assistant Vice Presi-dent, Nuclear, and is responsible for providing support services to the Nuclear Group in the areas of Records Management, Systems and Procedures, Document Control, and Computer Systems Management. The Information Management Director also assures that the site systems and procedures are developed and implemented in accordance with management policies and serves as the focal point for interaction with Centerior Nuclear Informa-tion System.

NUCLEAR HEALTH AND SAFETY DIRECTOR The Nuclear Health and Safety Director reports to the Assistant Vice President, Nuclear ard is responsible for the development of emergency planning preparedness, environmental monitoring and compliance, and industrial safety and hygiene programs to support Davis-Besse plant operations.

D-B SPECIAL PROGRAMS MANAGER The Special Programs Manager reports to the Assistant Vice President, Nuclear, and is responsible for developing and managing special adminis-trative tasks as requested by the Assistant Vice President, Nuclear.

QUALITY ASSURANCE DIRECTOR The Quality Assurance Director is responsible for the development, maintenance, and monitoring of the Nuclear Quality Assurance Program.

He reports directly to the Vice President, Nuclear and is responsible for advising the Plant Manager and cognizant corporate management of the Quality Assurance Program's effectiveness. The Quality Assurance Director 6 is independent from cost and scheduling considerations when opposed to safety considerations, and is free of non-QA related duties. The Quality Assurance Director is authorized to identify quality problems; initiate, recommend, or provide solutions, through designated channels; verify implementation of solutions; and verify conformance to established quality requirements.

The Quality Assurance Director has been given the authority by the Executive Vice President to suspend (stop work) any activity, except reactor operation, not in conformance with specified requirements and/or provisions of the NQAM. Conditions adverse to quality that appear to 5 warrant suspension of reactor operation, including startup or power generation are immediately reported to the Plant Manager. The Quality Assurance Director is responsible for the adequate staffing of the QA organization to ensure that the requirements of this NQAM are effectively implemented. The qualification requirements for the Quality Assurance Director are specified in the Nuclear Mission Policy and Organization Manual which is approved by the Vice President Nuclear.

QUALITY CONTROL MANAGER The Quality control Manager is responsible for managing the Quality control staff in the quality review, inspection, and examination of Q, Fire Pro-tection, ASME Code, and selected balance of plant structures, systems, and components to assure compliance to procedural, drawing, and engineering specification requirements. This responsibility includes the inspection of welding to verify it meets ASME, National Board, and State of Ohio requirements in addition to coordinating the activities of the Authorized Inspection Agency to ensure Code compliance.

ENGINEERING ASSURANCE MANAGER The Engineering Assurance Manager reports directly to the Quality Assur-ance Director and is responsible for managing the Engineering Assurance Staff in the review of engineering related activities to assure that the appropriate quality requirements have been specified and adhered to in the design and procurement process. He is also responsible for the review and approval of procurement documents, engineering specifications, vendor quality assurance programs and procedures, control of the Approved Vendors List providing external audits and surveillance activities and 6 receipt inspection.

The Quality Systems Manager reports directly to the Quality Assurance Director and is responsible for supervising the Quality Systems Staff in the development and incorporation of revisions in the Quality Assurance Program, procedures development and review, QA Division training and trend analysis.

DAVIS-BESSE PLANT MANAGER The Davis-Besse Plant Manager reports directly to the Vice President, Nuclear and is responsible for administering and directing all operation, maintenance, planning, and chemistry and radiological control activities in 16 conjunction with the continuous, efficient operation of the Davis-Besse Nuclear Power Station.

This is accomplished by assuring that chemistry parameters are identified for appropriate plant systems; that goals, administrative limits, warranty limits, and Technical Specification Limits are established; that plant procedures properly address chemical parameters, '

needs and concerns; that sample and analysis frequencies are specified; and that corrective actions for nonconforming conditions are specified.

RADIOLOGICAL CONTROL SUPERINTENDENT F The Radiological Control Superintendent is responsible for maintaining overall radiological control programs for work or activities associated with radiation or radioactivity. This responsibility also includes the development, management and revising as necessary, Radwaste Management Program plans, including the generation, segregation, processing, 6 disposition, recording and reporting of all waste materials with due regard to ALARA. ,

NUCLEAR PROJECTS DIRECTOR The Nuclear Projects Director reports to the Vice President, Nuclear, and is responsible for directing project management and major activities in support of Davis-Besse operations. This includes maintaining systems for project control, prioritization and allocation of resources; maintaining long range planning, scheduling and capital cost / budget controls; direct-ing field implementation of all facility modifications; and coordinating ,

the planning, engineering and implementation efforts for nuclear project activities. He also has financial responsibility for all Nuclear Group capital projects, and assists the Vice President, Nuclear, with operating budgets and financial control.

NUCLEAR MATERIALS MANAGER ,

DESIGN ENGINEERING DIRECTOR The Design Engineering Director reports to the Engineering General Manager and is responsible for directing and managing the performance of engineer-ing, safety evaluations, and accident analyses relative to modifications for Davis-Besse to assure optimum safety, availability, and reliability.

He also provides engineering support to address non-routine technical issues related to the operation and maintenance of Davis-Besse.

Ihe System Engineering Director reports to the Engineering General Manager and is responsible for minimizing Davis-Besse forced outage and lost capacity by providing systems engineering services to ensure proper installation, operation, maintenance, testing, and problem resolution for optimum system performance and reliability.

FIRE PROTECTION COMPLIANCE MANAGER The Fire Protection Compliance Manager reports to the Engineering General Manager and is responsible for the formulation, implementation, and periodical assessment of the effectiveness of the Fire Protection Program.

EQUIPMENT ENVIRONMENTAL QUALIFICATION MANAGER 5

The Equipment Environmental Qualification Manager reports to the Engineer-ing General Manager and is responsible for establishing an Environmental Qualification Program for electrical equipment in accordance with 10CFR50.49 to assure that the equipment will operate on demand in order to meet performance requirements under applicable service conditions.

NUCLEAR LICENSING DIRECTOR The Nuclear Licensing Director reports to the Vice President, Nuclear, and is responsible for directing and coordinating the activities related to licensing and interfacing with the NRC.

NUCLEAR LICENSING MANAGER The Nuclear Licensing Manager reports to the Nuclear Licensing Director and is responsible for directing and coordinating activities in obtaining, maintaining and revising licenses and permits required for the operation of Davis-Besse. He also coordinates activities directly involving the NRC.

NUCLEAR TRAINING DIRECTOR The Nuclear Training Director reports directly to the Vice President, Nuclear, and is responsible for the development and implementation of nuclear training programs to ensure compliance with regulations, commit-ments and industry standards.

NUCLEAR TRAINING PROGRAMS MANAGER The Nuclear Training Programs Manager reports to the Nuclear Training Director and is responsible for implementing the programs of the Nuclear Training Division by coordinating the daily operation of Nuclear Training in conformance to regulations, procedures and industry standards.

NUCLEAR SUPPORT TRAINING MANAGER The Nuclear Support Training Manager reports to the Nuclear Training

Director and is responsible for daveloping and implementation of the General Employee, Radiological Controls, Respirator, Security, Technical Staff and Managers, Har.ardous Chemical, Fire Watch and Nuclear Group Procedure Training Programs in an effective and economical manner.

INDEPENDENT SAFETY ENGINEERING DIRECTOR The Independent Safety Engineering Director reports directly to the Vice President, Nuclear, and is responsible for providing technical expertise on-site to allow for systematic and independent assessment of plant l6 activities. Additional responsibilities include surveillance of plant operations and maintenance activities to provide independent techni-cal verification that these activities are performed correctly and inves-tigation of plant incidents which may affect the underlying assumptions in the USAR Accident Analysis. The ISE Group functions independently from 6 other Nuclear Group activities.
