F APPENDIX A Conax Buffalo Corporation Docket No. .99900323/84-01 A

NOTICE OF NONCONFORMANCE Based on the results of an NRC inspection conducted on September 24-28, 1984, it appears that certain of your activities were not conducted in accordance with NRC requirements.

Criterion V of Appendix B to 10 CFR Part 50 states: " Activities affecting quality shall be prescribed by documented instructions, procedures, or drawings, of a type appropriate to the circumstances and shall be accomplished in accordance with these instructions, procedures, or drawings. Instructions, procedures, or drawings shall include appropriate quantitative or qualitative acceptance criteria for determining that important activities have been satisfactorily accomplished."

Nonconforrances with these requirements are as follows:

A. MS90 Calibration Record Logs at the Veeco Helium Leak Detector Control Consoles required recording of the serial numbers of the equipment calibrated.

Contrarytotheabove,twoMS90"RecordLogs"didnothavet5eserial numbers recorded on them. These " Record Logs" covered the periods September 21, 1984 thru September 25, 1984 and Septeniber 12, 1984 thru September 21, 1984.

B. Paragraph 1 of the Conax " Procedural Memorandum #3," dated April 14, 1982, states in part, "a ' Notice of Anomoly (sic)' should be instituted, on Form NPTL 82-2, when any Type Test Program Plan or Procedure Accept-ance Criteria is exceeded ...."

Contrary to the above, a notice of anomaly was not instituted when three out of four test specimens failed while undergoing qualification testing to Conax IPS-1086, Revision C, section 6.6.

C. "Quan. Rej." section of Material Otsposition Report (MDR) No. 27546 indicates that there were nine nonconforming pieces to be accounted for on MDR 27546. The defects and disposition sections require all nine pieces to be scrapped.

Contrary to the above, the M.R. Summary section of the MDR 27546 indicated that only three pieces were scrapped.
