Dear Mr.Morley:

This notification is made pursuant to Cook Nuclear Plant's NPDES Permit MI0005827, Part II, A.5.On November 8, 1997, during continuous chlorination of the Service Water Systems, plant personnel inadvertently discharged a portion of chlorinated Service Water to Outfall 002 for approximately nine hours without performing the required sampling and analysis for Total Residual Oxidant.These samples were missed due to a valve lineup error made during the startup of continuous chlorination after a short shutdown for general maintenance on the Circulating Water System.Shortly after the lineup was corrected, a sample was taken from the Outfall 002 discharge sample point at 10:15 a.m.on November 8, 1997.The sample measured 0.07 ppm Total Residual Oxidant (TRO), which exceeded the 0.036 ppm maximum discharge limit.The condition was discovered during a tour by plant personnel at which point the chlorination system was shut down November 8, 1997 at-09:00 a.m.Shortly after the discovery, the valve was repositioned to prevent discharge from Outfall 002 and direct all the Service Water to the Outfall 001, where circulating water dilution is present.Continuous chlorination resumed November 8, 1997 at 09:45 a.m.A briefing was held by the plant personnel involved with chlorination to emphasize the importance of checking the Circulating Water System valve lineup prior to commencing chlorination as required in their procedure.

The chlorination procedure includes steps to ensure that proper valve lineups have been performed to avoid NPDES exceedences.

Mr.Fred Morley Page 2 November 12, 1997 The Service Water System flows have been calculated to be approximately 24.MGD per unit.This is a slight portion of the normal circulating water flow of 1415 MGD for Outfall 002.This discharge caused no detrimental effects to the environment due to the small discharge flow and the large discharge piping (18'ia x 0.5 miles long).Normal water column demand for chlorine would have reduced the concentration to<.001 ppm by the time the flow reached Lake Michigan.Please contact me at (616)465-5901, ext 1153, with any questions you have.Sincerely, John Garison Environmental Compliance Supervisor
