ThefollowingnotificationisbeingperformedperNPDESPermitMI0005827,PartII.SectionA.6.,SpillNotification.Thespillwaspreviouslyreportedvia.telephonecallstoMr.GregDanneffeloftheMichigan,DNRPlainwellofficeat2:00p.m.andMr.Carrollofthe'ationalResponseCenterat5:10p.m.,onAugust29,1995(Reportg305645.)Thespilloccurredatapproximately8:30a.m.onAugust-.29,1995,whilediverswereusinghydraulicbrushestoremovezebramusselsfromtheCookNuclearPlant'sscreenhouseintakeforehaywalls.Thehydrauliclineburst'ndreleasedapproximately2gallonsof,hydraulic.'iltotheintakeforehay.Thediversimmediatelyshutdownthehydraulicscrubbingunitandremoveditfromthewater.Oilabsorbenthoomsweredeployedintotheforebaytocleanupthesurfaceoilsheen.SamplesoftheUnit2dischargeindicatednotraceofoilinthesample.PlantpersonnellaunchedaninflatableboattoinspectthedischargeforsignsofanoilsheenonLakeMichigan.Thedischargeplumewastraversedandverysmallwispsofoilwerenotedextendingsouthofthedischarge.Thelakewasverycalmduringtheinspection.895100b0044951002PDRADOCK05000315PDR page3Mr.FredMorleyHydraulicOilSpillIcertifyunderpenaltyoflawthatIhavepersonallyexamined.andamfamiliarwiththe.informationsubmittedonthisandallattacheddocuments;andbasedonmyinquiryofthoseindividualsimmediatelyresponsiblefor-obtainingtheinformation,Ibelievethesubmittedinformationistrue,accurateand.complete.Iamawarethattherearesignificantpenaltiesforsubmittingfalse,information,includingthepossibilityoffineandimprisonment.A.A.BlindSiteVicePresident
