DonaldC.CookNuclearPlantNPDESPermitNo.MI0005827IThiswrittennotificationisprovidedperNPDESPermitMI0005827,PartII,SectionA.6,SpillNotification,whichrequiresthatallspillstothesurfaceorgroundwatersofthestatebereported.CookNuclearPlanthasinstalledatemporary,skid-mounted,ice-makingsystemtoreplenishtheboratediceintheplant'sicecondensers.Theskidhasbeenprovidedwithaspillcontainmentsystem.OnJune23,1998,at1900hrs,theice-makingsystem'sdischargeline,containingaweaksodiumtetraboratesolution,developedaleakandoversprayedthecontainment,spillingapproximately50gallonstotheground.Wecalculatethatapproximately0.7lbs.ofboronand0.8lbs.ofsodiumweredischargedtotheground.Whentheleakwasdiscovered,thesystemwasshutdownandtheleakwasstopped.Theleakwasrepairedpriortorestartingthesystem.Thecauseoftheleakwasexcessivevibrationandwearagainstthedischargeline.Thedischargelinesupportshavebeenredesignedtopreventexcessivewearontheline.OnJune26,1998,at1410hrs,the.ice-makingsystem'sdischargeline,containingaweaksodiumtetraboratesolution,wasaccidentallycutduringworktoimprovethesystemdesign.Thecutleakedapproximately30gallonstotheground.Wecalculatethatapproximately0.5lbs.ofboronand0.5lbs.ofsodiumweredischargedtotheground.Thelinewasreplacedandthesystemwasrealignedtopreventfurtherfailures.Thegroundwherethespilloccurredwasexcavatedandproperlydisposed.Ifyouhaveanyquestions,pleasecontactmeat(616)465-5901,ext.1153.Sincerely,JohnP.GarisonEnvironmentalComplianceManagerItlm
