was found inoperable. This event constitutes operation in a degraded mode l
of LCO 4.4.1. Table 4.4-3 and is reportable per Fort St. Vrain Technical l                                      Specification AC 7.5.2(b)2.
EVENT DESCRIPTION:
i l                                      During performance of a scheduled test to verify plant protective system indicator lamp operability, when the reactor was in a shutdown condition, it was found that the lamps on one channel of 1D helium circulator drain mal-l                                      function (XDIS-21328) would not light. Further invertigation found that if
}                                      a drain malfunction had occurred, the channel would not have tripped. Tech-1                                      nical Specification LC0 4.4.1 requires that an inoperable channel be tripped.
!                                      Since it cannot be established when this channel became inoperable, operation
]
in a degraded mode of LCO 4.4.1 must be assumed.
i e                                      CAUSE l                                      DESCRIPTION:
The cause of the channel inoperability was a blown fuse in ene channel trip                                                                          .
!                                      circuitry of a model SW-2D switch module. The circuit design is such that
,                                      with this fuse blown, the channel is made inoperable with no indication of its status available to the operator. No reason for the fuse blowing could be found. It was replaced and has remained operable since then.
i                                      CORRECTIVE ACTION:
The blown fuse was replaced anu the channel was returned to service.

;                                      Public Service Company Nuclear Project Department has been requested to evaluate the circuit design to determine the feasibility of providing the operator some indication of a blown fuse in similar modules.
