a) G02-79-54, dated March 28, 1979 b) G02-79-89, dated May 9, 1979 c) G02-80-311, dated December 31, 1980 d) G02-81-565, dated December 30, 1981 The Washington Public Power Supply System herewith submits five (5) copies of the Preservice Inspection Summary Report designated Volume 4 of the Preservice Inspection Program Plan. Five (5) copies of the Program Plan and Amendments 1, 2 and 3 were originally submitted to you via the refer-enced letters.

The Summary Report documents the =-Supply System's completion of approxi-mately 98% of the volumetric and surface examinations committed to in the Preservice Inspection Program Plan. Also submitted as part of the Summary Report are 23, NIS-2 Owner's Report of Repair or Replacement forms for repairs or replacements completed to March 31, 1983.

In addition to documenting the volumetric and surface examinations and repairs and replacements, the Summary Report includes Request for Relief PSI-2-001. This section of the report documents all areas the Supply Sys-tem found that could not be fully examined to the ASME Section XI Code.

The Request for Relief contains only a few welds, all of which received a partial code examination. It is anticipated that no additional welds will be added to this request for relief.

Mr. A. Schwencer, NRC Page Two A supplement to this Summary Report will be submitted within 90 days after commercial operation. This supplement will contain a summary of the results of the remaining surface, visual and volumetric examinations.

Also contained in this supplement will be NIS-2 Owner's Reports for all repairs and replacements completed from March 31, 1983 to commercial operations and the NIS-1 Owner's Data Report for the Preservice Inspec-tion.
