The subject of this notification concerns a by-pass discharge on March 1, 1995, from the turbine room sump (TRS)through emergency over flow piping into the intake forebay to Lake Michigan.The cause of the discharge was due to a failure of the regulating valve (DRV-710)to open when signaled.The valve was eventually opened by defeating the air regulator to the valve and manually increasing the air pressure, forcing the valve to open.An action request has been initiated to repair DRV-710.Until DRV-710 is repaired, turbine room sump releases are being controlled by a manual isolation valve.The waters are sampled for pH and batch released via procedure 12-OHP 4021.062.001.

The pH range of the TRS water during the inadvertent discharge to the lake was 8.5 to 9.0.Outfalls 001 and 002 were analyzed for pH and outfall observations were performed at 0830 on March 1, 1995.The pH for outfall 001 water was 8.2.An outfall observation of 1 was made, meaning that the water was free of any unusual characteristics of the discharge, i.e., unusual turbidity, color, oil film, floating solids, foams, settleable solids or deposits.The pH for outfall 002 was 7.9, and an outfall observation of 1 was also made.The maximum estimated flow to the intake forebay was 0.016 million gallons.Five of the plant's seven circulating water pumps were on at the time of the by-pass.As required by the Cook Nuclear Plant National Pollutant Discharge Elimination System (NPDES)Permit No.MI6005827, written 9503270141 950320 PDR ADOCK 05000315 S PDR
