The most recent reactor trip of Sequoyah (SQN) Unit 2 which occurred this morning, being one of a series, raises concerns about operations at Sequoyah.
As we discussed this morning, SQN 2 should not re-enter mode 2 until TVA has conducted an in-depth review of the performance of the unit and met with senior management of the Office of Speciai Projects to discuss your findings.
The review sho~uld include an analysis of the entire series of reactor trips with emphasis on post trip review, causal analysis, correcti_ve actions taken, and a determination of why corrective actions have not been effective in preventing subsequent trips.
The time and place of the meeting is contingent on staff availability and TVA readiness to discuss the analysis.
