During the surveillance interval following the performance of Class I shock suppressors (snubbers) inspection test, two snubbers were determined to be inoperable. The reactor was taken critical for training purposes and this was the only reactor operation which occurred during the surveillance in te rval. However, since LCO 4.3.10 is applicable to all modes of reactor operation, except shutdown or refueling, this is being interpreted as oper-ation under a degraded mode of LCO 4.3.10 and is being reported per Fort St. Vrain Technical Specification AC 7.5.2(b)2.
EVENT DESCRIPTION:
While the reactor was in a shutdown condition, two snubbers were determined to be inoperable.
The hydraulic oil reservoirs on hydraulic oil snubber, HOS-29, and hot reheat snubber, HRS-357, were found below the snubber valves / cylinders.
CAUSE DESCRIPTION:
The rotation of the reservoirs is being attributed to personnel performing work on equipment near the snubbers and using the snubber units as stepping p la tfo rts . This, in turn, caused the lock nuts on the snubbers to loosen and allcwed the reservoirs to rotate.
CORFECTIVE ACTION:
The snubbers were returned to the proper positions.
Surveillance test SR 5.3.8a-x has been revised to include a snubber ori-entation verification.
Personnel have been reminded that plant piping and installations are not to be used in place of ladders in order to gain access to equip =ent.
