On October,1980, an unsampled gas waste release was made contrary to the requirement of LCO 4.8.1. This is reportable per Fort St. Vrain Technical Specification AC 7.5.2(b)3.

EVENT DESCRIPTION:

An authorized release from a gas waste surge tank was to be made. The tank was sampled and analyzed in accordance with LCO 4.8.1, and the gaseous radio-active effluent continuous monitors were verified to be operable and opera-ting. During the valve lineup f or the release, a valve was inadvertently lef t open which diverted the gas waste surge tank contents to the gas waste vacuum tank.

The vacuum tank is protected against over-pressure by two parallel rupture discs and relief valves which discharge into the same line as a normal re-lease from the gas waste surge tanks. When the gas released from the gas waste surge tank over-press",rized the vacuum tank, a rupture disc ruptured and the associated relief valve lif ted, releasing the contents of the vacuum tank, which had neither been sampled nor analyzed as required by LCO 4.8.1.

The gaseous radioactive effluent monitors tested prior to the re lease did not indicate any radioactive release. However, an auxiliary monitor of greater sensitivity indicated a short term (less than 5 minutes) release above background that did not exceed the maximum permissible co< centration allowed by LCO 4.8.1. Calculations indicate a release of 0.01~ curies of noble gases through the reactor plant exhaust system.

CAUSE DESCRIPTION:

l The cause of this event was a broken valve stop which prevented the valve l f rem being closed fully. '

When alarms were received due to the over-pressurizing of the gas waste vacuum tank, the valve with the broken stop was closed, the correct valve lineup was established, and the release completed via the normal flow path.

The broken valve stop and the burst rupture disc were replaced.
