Thesubjectofthisnotification concernstheidentification ofaninfluentstreamtothestormwateroutfall001Sandareleaseofsodiumnitrite.Theofficebuildingheatingandairconditioning (HVAC)systemcondensate drainheaderdischarges tothestormwatersystematanunderground pointadjacenttotheofficebuilding.

Thedischarge consistsofHVACcondensate collected indrippansthatflowtoacommondrainheader.Theconnection ofthecondensate draintothestormwatersystemwasdiscovered asaresultofarecentsystemleak.OnMarch3,1994,itwasdetermined thattheofficebuildingHVACsystemwasleakingwatercontaining approximately 1,000mg/litersodiumnitrite(anadditiveintheHVACsystem)atacalculated rateof0.5gpm.Thefollow-up investigation revealedthattheleakbeganapproximately onFebruary22,1994,andcontinued untilaleakingreliefvalvewasfoundandrepairedonMarch7,1994.Aspreventive action,theofficebuildingHVACsystemwillcontinuetobetestedweeklyfornitriteswithanyunusuallossesreportedtoplantsupervision.
