Donald C.Cook Nuclear Plant NP DES Permit No.MI0005827 I This written notification is provided per NPDES Permit MI0005827, Part II, Section A.6, Spill Notification, which requires that all spills to the surface or groundwaters of the state be reported.Cook Nuclear Plant has installed a temporary, skid-mounted, ice-making system to replenish the borated ice in the plant's ice condensers.

The skid has been provided with a spill containment system.On June 23, 1998, at 1900 hrs, the ice-making system's discharge line, containing a weak sodium tetraborate solution, developed a leak and oversprayed the containment, spilling approximately 50 gallons to the ground.We calculate that approximately 0.7 lbs.of boron and 0.8 lbs.of sodium were discharged to the ground.When the leak was discovered, the system was shut down and the leak was stopped.The leak was repaired prior to restarting the system.The cause of the leak was excessive vibration and wear against the discharge line.The discharge line supports have been redesigned to prevent excessive wear on the line.On June 26, 1998, at 1410 hrs, the.ice-making system's discharge line, containing a weak sodium tetraborate solution, was accidentally cut during work to improve the system design.The cut leaked approximately 30 gallons to the ground.We calculate that approximately 0.5 lbs.of boron and 0.5 lbs.of sodium were discharged to the ground.The line was replaced and the system was realigned to prevent further failures.The ground where the spill occurred was excavated and properly disposed.If you have any questions, please contact me at (616)465-5901, ext.1153.Sincerely, John P.Garison Environmental Compliance Manager Itlm
