This notification is forwarded for your review pursuant to Appendix"B" of Donald C.Cook Nuclear Plant Environmental Technical Specifications.

The subject of this notification concerns the identification of an influent stream to the storm water outfall 001S and a release of sodium nitrite.The office building heating and air conditioning (HVAC)system condensate drain header discharges to the storm water system at an underground point adjacent to the office building.The discharge consists of HVAC condensate collected in drip pans that flow to a common drain header.The connection of the condensate drain to the storm water system was discovered as a result of a recent system leak.On March 3, 1994, it was determined that the office building HVAC system was leaking water containing approximately 1,000 mg/liter sodium nitrite (an additive in the HVAC system)at a calculated rate of 0.5 gpm.The follow-up investigation revealed that the leak began approximately on February 22, 1994, and continued until a leaking relief valve was found and repaired on March 7, 1994.As preventive action, the office building HVAC system will continue to be tested weekly for nitrites with any unusual losses reported to plant supervision.
