This report stated that all questionable welds joining CB&I Class D materials j hadL been visually examined and all welds found deficient as a result of these ;
inspections, had been repaired. -
While in the process of reviewing this item for- closure, it was found -that all the reports for these inspections were not available. An investigation was performed by.-both Ebasco and CB&I to determine whether all the inspections had- been performed and if .so, to locate the required documentation.
No further proof of the required inspections having been performed has resulted from these investigations. Therefore, it must be assumed that these inspections have not as yet been performed.
