emergency preparedness.

Results: One violation was identified - failure to provide training to

emergency response personnel in accordance with Emergency Plan ' Procedure

The inspection scope and findings were summarized on June 19, 1987, with

those persons indicated in Paragraph 1 above. The inspector described the l

areas inspected and discussed in detail the violation identified in {

Paragraph 5 below. In addition, the inspector discussed the Management j

Control System for maintaining training records. No dissenting comments j

were received from the licensee. The licensee did not identify as l

proprietary any of the material provided to or reviewed by the inspector i

during this inspection.

4. Changes to the Emergency Preparedness Program (82204) f

Pursuant to 10 CFR 50.47(b)(16); 10 CFR 50.54(q); ar.d 10 CFR Part 50,

Appendix E, Sections IV and V, this area was reviewed to determine whether i

changes were made to the program since the last routine inspection (May 1

1986) and to note how these changes affected the overall state of

emergency preparedness.

The inspector discussed the licensee's program for making changes to the

Emergency Plan and Implementing Procedures. The inspector reviewed the

licensee's system (delineated in Plant Administrative Procedure 01-S-02-2,

Nuclear Licensing Administrative Procedure No. 2.8, and Section 8.5 of the

Emergency Plan) for review and approval of changes to the Plan and

Procedures. The inspector verified that changes to the Plan and

Procedures were reviewed and approved by management. It was also noted

that all such changes were submitted to NRC within 30 days of the

effective date, as required.

Discussions with licensee representatives indicated that although no

significant modifications to facilities, equipment, or instrumentation

were completed since the last inspection, Plant and Corporate Management

have concurred on expansion plans for the Technical Support Center (TSC).

According to licensee personnel, construction is scheduled to begin during

August 1987 with a tentative completion date for October 1987. A licensee

representative indicated that construction will not impact or degrade the

normal operation and/or use of the TSC in the event of an incident. In (

addition to the TSC expansion, discussions with licensee personnel i

disclosed that a new Claiborne County Emergency Operations Center (E0C) is I

being built in Port Gibson, and the licensee's Rumor Control Facility was l

relocated from the MP&L Electric Building ir, Jackson to the SERI I

Headquarters Building located at One Jackson Place in Jackson.

The organization and management of the emergency preparedness program were

reviewed. Since the last inspection, changes were made to the emergency .

preparedness staff for both the Plant and Corporate Organization. Changes

'

to the corporate organization involved personnel and the reporting chain.

The position of Emergency Preparedness Coordinator was reassigned, I

including a change in title (" Supervisor, Emergency Preparedness"). In i

addition, the reporting chain for the Corporate Emergency Preparedness

Staff was revised from reporting to the Nuclear Licensing and Safety

Di rector, to a newly created position known as " Manager Emergency

Preparedness and Executive Assistant to 'li ce-Presi dent , Nuclear

Operations." The station change involved the first line supervisory

reporting chain. Although the aforementioned changes were made since the

last inspection, discussions with licensee personnel indicated that there

were no changes in responsibility for the plant or corporate staffs. The

inspector's discussion with licensee representatives also disclosed that

there had been no significant changes in the organization and staffing of

the offsite support agencies since the last inspection.

The inspector reviewed the licensee's program'for distribution of changes

to the Emergency Plan and Procedures. Document control records for the r

period June 12, 1986, to March 25, 1987, showed that appropriate personnel

and organizations were sent copies of Plan and Procedure changes, as

requi red.

No violations or deviations were identified. ,

l S. KnowledgeandPerformanceofDuties(Training)(82206)

Pursuant to 10 CFR 50.47(b)(15) and 10 CFR Part 50, Appendix E,

Section IV.F, this area was inspected to determine whether emergency

response personnel understood their emergency response roles and could

perform their assigned functions.

The inspector reviewed the description of the training program in

Section 8.2 of the Emergency Plan, Section 6.2 of the Emergency Plan

Procedure No.10-S-01-23, and interviewed members of the instructional

staff. Based on these reviews and interviews, the inspector determined 1

that the licensee had established a formal emergency training program, j

Records of training for personnel assigned to the on-call list for the

second quarter of 1987 were reviewed. The review involved training

documentation for the period March 1985 to June 1987. It was noted that  !

two members of the current on-call list, designated as Operations l

Coordinator, had not received the required training as specified in 1

Administrative Procedure 01-S-04-21, " Emergency Preparedness Training

Program." Both individuals failed to complete retraining for respiratory

certification in accordance with Administrative Procedure 01-S-04-10. In

l addition, one of the two individuals had not completed Radiation Worker II

l annual training. A licensee representative acknowledged this finding and

l indicated the two individuals would be removed from the on-call list ,

pending completion of all training requirements. This item is identified

I

l

as a violation of 10 CFR 50.54(q) which requires the licensee to follow an

Emergency Plan which meets the planning standards in 10 CFR 50.47(b).

Violation (50-416/87-16-01): Failure to provide decertification training

to personnel assigned to the station emergency organization.

In addition to the above finding, during the review of training records

!- for personnel assigned to the on-call list, the inspector noted that four

'

individuals assigned as TSC Communicators had no records to verify that a

respiratory fit test had been performed to satisfy respiratory

l decertification requirements in accordance with Administrative

Procedure 01-S-04-10. This matter was discussed with a licensee

representative who acknowledged this finding, but took exception to the

finding that training was not attended by personnel. According to the

licensee, decertification training was provided on March 4, 1987.

However, the completed training package to verify completion of the refit

test was not on recora. This item was also identified by the licensee's

Quality Assurance Group and a Quality Deficiency Report (QDP,) was issued

on June 15, 1987. The QDR (No. 266-87, Transmittal No. 87/1519),which '

was transmitted to -various plant personnel, indicated that an

investigation was .in progress to locate documentation. The QDR included

corrective action to preclude recurrence and the corrective action ,

completion date. A licensee representative (Manager, Plant Support)  !

concitted to removal of the four individuals from the on-call list and

retraining if the training package is not located. The licensee

representative further stated that the individuals' training qualification  ;

card was initialed as having completed the decertification on March 4,  !

1987. The inspector was provided with each individual's training ,

qualification card for review and verification. Although.the inspector

verified the dates on the training qualification cards, the licensee was .

informed that this matter is considered an inspector followup item. j

Inspector Followup Item (50-416/87-16-02): Provide documentation to  ;

verify completed respiratory training for personnel assigned as TSC l

communicators during the second quarter of 1987. .

The licensee was also informed that the mishandling of training records

which led to the QDR is considered an inspector followup item. 1

Inspector Followup Item (50-416/87-16-03): Maintain training records in

accordance with Plant Administrative Procedure 01-S-04-14.

According to training records, with the exception of the findings stated

above, all other training provided to key members of the onsite on-call

list was consistent with the Plan and Procedures. The inspector reviewed l

training records for key members of the licensee's offsite emergency )

organization. It was noted that several individuals had not completed

training in accordance with the description in the Emergency Plan and

l Emergency Plan Procedure. This matter was discussed with a licensee

representative who acknowledged this finding and indicated that training

would be completed by the date included in the MP&L-SERI Transition Plan

(June 30, 1987). The licensee was informed that this item will be

reviewed during a subsequent inspection.

Inspector Followup Item (50-416/87-16-04): Complete all training for I

offsite emergency organization personnel.-  ;

Documentation was provided to .the inspector regarding the training of j

offsite support agencies to verify that training was conducted during 1986

(June and August) and 1987 (June) in accordance with section 8.0 of the

Emergency Plan.

One violation and no deviations were identified.

6. Licensee Audits (82210)

Pursuant to 10 CFR 50.47(b)(14) and (16) and 10 CFR 50.54(t), this area

was inspected to determine whether the licensee had performed an

independent review or audit of the emergency preparedness program.

Records of audits of the program were reviewed. The records showed that l

an independent audit of the program was conducted by Audits Quality I

Assurance Group from February 23, 1987, to March 18, 1987, and was j

documented in Audit Report No. MNEP-87/01, dated April 23, 1987. This '

I audit fulfilled the 12-month frequency requirement for such audits. The

l audit records showed that the State and local government interfaces were

evaluated, and according to Quality Assurance personnel, State and local

government authorities were informed that findings concerning the State {

and local interfaces v.ould be available for review. Audit findings and j

recommendations were presented to plant and corporate management. A J

l review of past audit reports indicated that the licensee complied with the i

i

five-year retention requirement for such reports.

Licensee Emergency Plans and Procedures require critiques following

exercises and drills. Licensee documentation dated January 19, 1987, j

showed that a critique was held following the annual exercise in December

1986. The documentation showed that deficiencies were discussed in the  !

critique, and recommendations for corrective action were made.  !

The licensee's program for follow-up action on audit, drill, and exercise f 4

findings was reviewed. Licensee procedures required follow-up on

deficient areas identified during audits, drills, and exercises. The

inspector reviewed a sample of licensee records for February and March

1987, which indicated that corrective action was taken on identified

problems, as appropriate. The licensee had established a tracking system

known as the " Action Tracking System (ATS)," for use as a management tool i

in following up on actions taken in deficient areas. l

the inspector noted that the beta dose rate was obtained by a survey 1

containment. A review of licensee documentation (Design Change

Package No. 84/0231) and discussions with licensee representatives

disclosed that evacuation warning ?ights are being installed in l

combination with PA system for high noise areas of the plant. l
