SUMMARY
Scope: . This special, reactive, announced inspection was conducted in response
to the frequent spurious activations of the Grand Gulf Nuclear Station Alert
and Notification. System (ANS) during the period November 29, 1988 through
. January 12, 1989.
The inspection involved a detailed review of the ANS, its
operation and maintenance, and training of offsite support agency personnel
assigned to system monitoring and operation.
Inspection also included a review
.of applicable licensee administrative and emergency preparedness procedures,
and a walkdown of ANS activities. and control facilities in the States of
Mississippi (Port Gibson, Clairborne County) and Louisiana (St. Joseph, Tensas
Parish).
Results:

Alert Notification System (ANS) Status (82203)
The facility ANS consists of 43 sirens.
Thirty sirens are strategically
located throughout the Clairborne County, Mississippi area. The remaining
13 sirens are located in Tensas Parish, Louisiana.
The ANS is provided
with .two radio controlled activation systems. At the time of inspection,
the original activation system (single-frequency radio encoder) was
deactivated in the Clairborne County area, and the licensee immediately
changed over to the recently installed frequency shif t keyed (FSK) digital
activation sy tem.
Changeover of ANS control system, including the
13 sirens located in Tensas Parish, Louisiana, is scheduled for completion
during the current calendar quarter.

The frequent spurious activations of ANS sirens were essentially confined
to Clairborne County, during the above cited period.
The licensee
conducted a detailed review and inspection of the ANS, the individual
sirens involved, and the two operational ANS activation systems to
'
determine the cause(s) of the referenced incidents.
It was determined
that the single-frequency radio encoder activation system was the cause of
the incidents as a result of apparent receotion of spurious radio signals
generated from sources outside the ANS system. Accordingly, the licensee

. promptly ' initiated use of the FSK-digital activation system, and-
'
concurrently deactivated the single-frequency radio encoder system.
No violations or deviaitons were identifed.
3.
Review and.0bservation of ANS Operation (82203)
The inspector conducted a detailed review and inspection of the ANS
including the following: (1) the radio controlled activation systems
including design, operation, and maintenance thereof; (2) walkdown of
primary and backup locations of ANS activation systems; .(3) observation of
system-testing, monitoring (and operation; and (4) discussion of training
of .offsite support agency s) personnel assigned to monitoring and
operation of the ANS.
The Clairborne County ANS activation system is
located in the County Civil Defense building in Port Gibson, Mississippi.
The inspector observed routine use of the FSK-digital system in conducting
silent tests or polling of the entire ANS, including the sirens in Tensas
Parish, Louisiana.
Assigned personnel appeared to be adequately trained
regarding ANS monitoring, operation
testing, and interpretation of

computer data docuinenting the operational status of each siren in the
Alert Notification System.
A backup FSK digital system console for the
subject area was located at.the Clairborne County Sheriff's Office in Port
Gibson.
Inspection disclosed that the console was fully operational.
Additionally, trained personnel were available, on a 24-hour basis, to
operate the FSK digital activation system if required.
Inspection of- the primary ANS activation system located at the Tensas
Parish Sheriff's headquarters in St. Joseph, Louisiana, disclosed that
the single-frequency radio encoder and the FSK digital activation systems
were in place and fully operational.
Operational testing of the FSK
digital system also demonstrated that the entire ANS, including
Mississippi, could be polled and activated from that location, if
required.
Inspection also disclosed that the assigned staff appeared to
be adequately trained regarding system monitoring, testing, and operation.
The single-frequency activation system at this' location was scheduled for
deactivation and removal during the current calendar quarter.
Inspection
disclosed that concurrent with on-going monitoring and testing of the new
ANS activation system, provisions for additional protection of the radio
!
frequency to each siren in the ANS were being made which included private
line radio decoders.
Installation of the decoders was factored into the
previously referenced completion schedule for the ANS activation system
modification.
The projected completion of all required modifications and
operational acceptance of the ANS FSK dig) ital activation system will be
tracked as an Inspector Followup Item (IFI .

4.
Review and Audit of ANS Procedures (82203)
The inspector conducted a detailed review and audit of applicable Service
Energy Resources, Inc. (SERI) Emergency Preparedness Administrative
Procedures (EPAPs) and selected Grand Gulf Nuclear Station (GGNS)
Operations Manual and Administrative Procedures developed to ensure
operation of the ANS consistent with Emergency Plan requirements.
As a consequence of the frequent spurious activations of the ANS in
Clairborne County and the resulting NRC and local public concern, GGNS
Administrative Procedure 01-S-06-5 (Incident Reports / Reportable Events)
was revised to include reporting)of ANS incidents under NRC regulatory
requirement 10 CFR 50.72(b)(2)(vi . Accordingly, the licensee will' notify
the NRC as soon as practical but, at least, within four hours of ~ the
occurrence.
Consistent with the referenced procedural revision,
SERI EPAP-201 (Emergency Preparedness Program) which provides guidance on
monitoring routine ANS testing, was also revised to include unplanned
siren activations.
Revision of the latter procedure was designed to
ensure that tne Control Room is notified of ANS incidents in accordance
with piant Administrative Procedure 01-S-06-5.
Additionally, the Station
Emergency Response Organization coordinates with SERI Corporate
Communication
in development and subsequent release of news bulletins
addressing ANS incidents.
Review and inspection of training of offsite support agencies and
i
respective personnel assigned to operation of the ANS was also conducted.
Section 2.8.1 (Training, Drills, and Exercises) of EPAP-201 states, in
part, that all emergency preparedness training of offsite support agencies
in accordance with requirements defined in EPAP-203 (Emergency
Preparedness Training, Drills, and Exercise) shall be provided, or ensured,
and documented by the Emergency Preparedness staff.
As previously
discussed, offsite support agency (s) personnel assigned to operation of
the ANS appeared to be adequately trained regarding the recently installed
FSK digital activation system.
Inspection disclosed however, that
training of the subject personnel regarding operation of the ANS was not
documented by the licensee as required.
Although the above referenced
'
personnel appeared to be adequately trained, the unavailability of
training records and related documents precluded a valid assessment of the
adequacy of training completed and the frequency thereof.
As a result,
the inspector could not determine compliance with the Emergency Plan and
respective training requirements. Technical Specification 6.8.1.e requires,
in part, that written procedures shall be implemented and maintained
covering Emergency Plan implementation.
This finding was fully discussed
with cognizant licensee representatives prior to and during the exit
interview.
The inspector informed licensee representatives that failure
to follow the referenced procedure (Section 2.8.1 of EPAP-201) constituted
a violation.
The licensee acknowledged this finding and offered no
dissenting comments.
