3. IF reactor building entry is required to restore C@t monitor equipment, _THEN
a. D . PATCH the following minimum manpower for performance of this procedure:

I b. OBTAIN the following keys from the S05 Key Locker (1) ASSD Equipment Cabinet Key #148 (1) ASSD Flashlight Tool Rox Key #160 AND PROCURE the following equipment from the ASSD Equipment Cabinet.

For Reactor Building (1) Sound powered phone (1) Flashlight (1) Copy of this procedure (3) Remote Shutdown Keys, Serial T112 (1) Security Access Key (1) Twenty-five foot sound powered phone extension cord.

c. USE appropriate figures in this procedure to provide access / egress routes, equipment and comunication locations. (Normal comunications may not be available.)

4 IF Nuclear Service Water to Vital Header' Valve,1-SW-V117 CANNOT_

E operated f rom the RTGR, THEN flanually OPERATE l-SW-V117.

5. IF Nut. lear Service Water Supply Valve,1-SW-V105 CANNOT be operated from the RTGB,_THEN Manually OPERATE l-SW-V105.

8 IF operation of HPCI Steam Supply Inboard Isolation Valve, 1-E41-F002 is required AND pcwer is NOT available f rom MCC IXD, THEN

a. PLACE the circuit breaker control switch in the OFF position for HPCI Stean Supply Inboard Isolation Valve, 1-E41-F002, at tiCC IXD compartnent DW1.
b. PLACE the circuit breaker control switch in the ON position for HPCI Stean Supply Line Isolation VaTv'e, 1-E41-F002, ( ASSD FEED) at MCC IXC compartment DSI.

c, IF 1-E41-F002 is required to be Open, THEN PLACd the ISSD Keylock Control Switch in the OPEN position, at MCC IXC compartment DSI.

a. PLACE the circuit breaker control switch in the OFF position for HPCI Turbine Er.haust Vacuun Breaker Valve,1-E41-F079, at f1CC 1XB compartment 000,
b. PLACE the circuit breaAer control switch in the ON -

position for HPCI Turbine Vacuun Breaker Valve, -

1-E41-F079 ( ASSD Feed) at MCC IXC compartment DT2.

c. IF 1-E41-F079 is required to be Open, THEN PLACE the HSD Keylock Control Switch in the OPEN position, at MCC IXC compartment DT2.
d. E l-E41-F079 is required to be Closed,_THEN PLACE the ASSD Keylock Control Switch in the CLOSE position, at MCC IXC compartment DT2.
