ENCLOSURE 1 NOTICE OF VIOLATION Virginia Electric and Power Company Docket Nos. 50-338 and 50-339 North Anna License Nos. NPF-4 and NPF-7 During the Nuclear Regulatory Commission (NRC) inspection conducted on June 2 -

July 6,1986, violations of NRC requirements were identified. The violations involved failure to require a review of Design Change Acceptance Tests for Technical Specification requirements and failure to perform a written safety evaluation. In accordance with the " General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1985), the violations are listed below:

A. 10 CFR 50 Appendix B, Criterion XI, Test Control, states in part that a. test program shall be established to assure that all testing required to demonstrate that structures, systems and components will perform satis-factorily in service is identified and performed in accordance with written test procedures which incorporate the requirements and acceptance limits contained in applicable design documents. Also, the test results shall be documented and evaluated to assure that test requirements have been satis-fied.

Contrary to the above, as of July 6,1986, the licensee did not have a formal program requiring acceptance tests for design change procedures to be reviewed for Technical Specification requirements of a formal method to provide additional testing to meet Technical Specification requirements prior to the modified equipment being declared operational.

This is a Severity Level IV violation (Supplement I) for Units 1 and 2.

B. Technical Specification 6.8.1.a. requires the licensee to implement approved procedures covering the areas recommended in Appendix "A" of Regulatory Guide 1.33, Revision 2, February 1978, which includes administrative procedures for equipment control.

Administrative Procedure 14.1, Jumpers (Temporary Modifications), requires a written safety evaluation for jumpers installed in systems that are safety related or installed in systems, components or equipment which are described in the Updated Final Safety Analysis Report (UFSAR). This procedure complies with the 10 CFR 50.59 requirement which requires the licensee to maintain records of changes in the facility, including a written safety evaluation which provides the basis for the determination that the change does not involve an unreviewed safety question, B 860729 k8)QK05000338 G

Contrary to the above, Administrative Procedure 14.1 requirements were not

. fully implemented in that no written safety. evaluation was performed for an electrical jumper installed on January 2, 1986. Jumper N-1-1165 was installed for the purpose of removing from service the IH Emergency Diesel Generator Control Room Annunciator.

This is a Severity Level IV violation (Supplement I) for Unit 1 only.

Pursuant to the provisions of 10 CFR 2.201, Virginia Electric ' Power Company is hereby required to submit to this Office within 30 days of the date of the letter transmitting this Notice a written statement or explanation in reply including for each violation: (1) admission or denial of the violation, (2) the reason for the violation if admitted, (3) the corrective steps which have been taken and the results achieved, (4) the corrective steps which will be. taken to avoid further violations, and (5) the date when full compliance will'be achieved. Where good cause is shown, consideration will be given to extending ~the response time.
