In accordance with Appendix A, Beaver Valley Technical Specification 3.7.15, "Fire Rated Assemblies", the following Special Report is submitted. This report is required when the conditions specified in Technical Specification 3.7.15.a are not satisfied.

During Beaver Valley's 6th Refueling outage, there were seven fire dampers that failed to fully close during their operational testing and could not be restored within the Technical Specification time limit of seven days. These dampers have been adjusted and returned to service after retesting except VS-D-260 and 285. Task Force evaluation has been initiated concerning these damper release mechanisms, latchec and general damper condition. Basically, the dampers have all been worked and tested, but a satisfactory pass rate has not been achieved prompting this further evaluation. These dampers will be returned to service after the completion of this evaluation and successful retesting.

There were no safety implications due to this event. A fire watch will be maintained in the affected area until all dampers are operable.
