Murley Regional Administrator United States Nuclear Regulatory Commission Region 1 Park Avenue King of Prussia, PA 19406 Gentlemen:

In accordance with Appendix A, Beaver Valley Technical Specification 3.3.3.1, " Radiation Monitoring Instrumentation," the following revision to a Special Report (NDISS1:2506, dated 7/29/85), is submitted. This report is required when the conditions specified in Technical Specification 3.3.1.1.6 are not satisfied.

On 7/20/85 at 1630 hours0.0189 days
0.453 hours
0.0027 weeks
6.20215e-4 months
, RM-MS-100B, Main Steam Monitor was declared out-of-service due to the receipt of a high alarm with normal background radioactivity present. This is nonconservative with respect to Technical Specification 3.3.3.1 and reportable per Technical Specification 3.3.3.1.b. This channel is one of three that monitors atmospheric steam dump valve and code safety relief valve discharge from the steam generator relief headers. After extensive troubleshooting, it was found that a loose triaxial connector between the detector and preamp board was the origin of the spurious readings. Replacement parts were installed and on 10/22/85 at 0320 hours0.0037 days
0.0889 hours
5.291005e-4 weeks
1.2176e-4 months
, RM-MS-100B was placed back in service. There were no safety implications to the public, as alternate monitoring was available during this time.

All similar monitors will be inspected for this type of problem during the performance of their next maintenance surveillance procedure.

Following the evaluation of any findings, the appropriate reporting

. . requirements will be fulfilled, if deemed necessary.

Very truly yours, Y o 'desa 9601160406 85111234 Wm. S. Lacey DR ADOCK 05 P M er v
