In accordance with Appendix A, Beaver Valley Technical Specifications 4.4.5.5.a, "Stean' Generators," the following Special Report is submitted. This report is required to document the results of the Inservice Inspection of the steam generator tubes, performed during the Fourth Refueling Outage.

On October 14, 1993, the inservice inspection of the tubing in steam generators 2RCS-  ;

SG21A (S/G A), 2RCS-SG21B (S/G B), and 2RCS-SG21C (S/G C) was completed. The number of tubes inspected is documented below.

One hundred percent (100%) of all inservice tubes in the three steam generators _ were inspected during the outage. Two tubes were removed from_ service due to tube degradation in excess of the Technical Specification limit of 40 percent through wall. Two additional tubes were removed from service due to denting.

Six Babcock & Wilcox (B&W) rolled plugs on the cold leg side of S/G 21C were replaced with Babcock & Wilcox Inconel 690 plugs, completing the replacemeut of all Inconel 600 B&W plugs at Unit 2.

A review of the inspection data coupled with related industry experience resulted in the precautionary plugging of thirty-four (34) tubes in 2RCS-SG21A, twenty-three (23) tubes in 2RCS-SG21B, and twenty-nine (29) tubes in 2RCS-SG21C. The tubes were plugged based on .

the indications received at the following locations: U-Bend, Support Plate, Free Span, and Top ,

of Tubesheet. Additionally, eight (8) tubes in 2RCS-SG21B were plugged following stabilization due to loose part indications received during steam generator secondary side foreign object inspections.
