In accordance with Appendix A, Beaver Valley Technical Specification, 3.3.3.1, " Radiation Monitoring Instrumentation", the following revision to a Special Report (NDISS1:2013, dated 2/24/84) is submitted. This report is required when the conditions specified in Technical Specification 3.3.3.1.b are not satisfied.

On 2/15/84, at 1635 hours0.0189 days <br />0.454 hours <br />0.0027 weeks <br />6.221175e-4 months <br />, Channels 7 and 9 of the Special Particulate Iodine, Noble Gas (SPING) Monitor for radiation monitor (RM-VS-110) were de-clared out-of-service due to the constant receipt of high radiation alanns with nonnal activity present. This is non-conservative with respect to the Technical Specification 3.3.3.1, and reportable per Technical Specification 3.3.3.1.b. These channels provide indication only and monitor the exhaust effluent of the Supplementary Leak Collection and Release System. Al ternate preplanned methods are in effect until these channels are returned to service.

Investigations to detennine the cause of the erroneous signals were conducted, but yielded no detennination of the root cause. The vendor (Eberline Instru-ment Corporation) was contacted and was scheduled to visit the site on 3/1/84 to assist in the troubleshooting process and the restoration of these channels. I An Eberline Instruments Service Engineer was brought in and found problems in the control tenninal's software and the background channel parameter files.

After manipulation of the bubble memory and re-editing of the parameter files, channels 7 and 9 were returned to service. Wrltten instructions on the para-meter files were left by the Eberline Representative in the event that a fdture prcblem arises.

On February 15,1984 at a on the Control Teminal (CT-1)pproximately 1500 hrs, Hi Leak for VS-110 (Supplementary alamsCollection were indicated and Release System Monitor). After verifying the readings were incorrect, channels 7 and 9 (medium and high range noble gas technical specification channels) were tagged 0.0.S. and MWR #840413 was written.

Corrective Actions Taken:

An Eberline Instruments Service Engineer was brought in to rectify the problems, which were found to be from the CT-1 software and the background channel parameter file.

After manipulating the bubble memory and eediting the parameter files, channels 7 and 9 returned to nomal. Written instructions were lef t by the Eberline representative. .

Corrective Actions That Will Be Taken:

i A copy of the channel 7, 8 (background channel) and 9 parameter files for each of the three SPING monitors will be available in the Control Room's CT-1 notebook. Instructions will be provided on their use in the event of another problem.

Corrective Actions That Should Be Taken:

The station does not have a complete spare parts kit for the SPING monitors.

A spare parts list was submitted to Procurement in 1982. These parts are necessary for the station to perfom its own maintenance on the monitors, and should be acquired as soon as possible.
