Special Report United States Nuclear Regulatory Commission Document Control Desk Washington, DC 20555 Gentlemen:

In accordance with Beaver Valley Unit i License Condition 2.C.5, the following Special Report is submitted. The report identifies an event where a degraded condition was identified involving the Train B Charcoal Filter Bank Fire Suppression System.

On 1/11/93 a test was performed to verify operability of the Train 11 Charcoal Filter Bank Fire : Suppression System.

This' test verifies the operation of the Supplementary Leak Collection and Release System (SLCRS) Main Charcoal Filter Bank Fire Suppression System by demonstrating that each nozzle can pass unobstructed '

flow. To perform the sprinkler nozzle test, the charcoal cells are removed and air is passed through each nozzle by feeding station air through a pressute regulator. -Tissue held in front of each nozzle must be displaced by the air flowing through in order for the test to be satisfactory.

The results of the test indicated that eighty (80) of the ninety (90) n~ozzles were.

clogged with a white scale material. The nozzles were removed and cleaned, new strainers were installed and unobstructed flow was verified on 1/14/93.

February 9,1993 ND3MNO:3411 Page 2 The cause for this event was the use of an undersized strainer in the nozzle assemblies. The strainers were installed in 1988, as a result of previous problems associated with nonle clogging (reference 1.IIR 87-021-01). The strainer design was to utilire a 24 mesh (0.028 inch) monel screen strainer. A purchasing / ordering error resulted in the receipt and installation of a 100 mesh strainer in the nozzle assemblies.

The 100 mesh strainers have been removed from both Train A and B Filter Bank Fire Suppression Systems and replaced with 24 mesh strainers.

There were ininimal safety implications as a result of this event. This potential loss of one train of the SLCRS filter banks due to a fire has been previously evaluated.

This evaluation considered several scenarios and the resulting radiological consequences.. The worst case scenario involving a Design 11 asis Accident with a resulting charcoal filter Fank fire results in the maximum offsite dose being greater than 10CFR20 limits, but well below the 100CFR100 limit of 300 rem.

Additionally, the Train A Charcoal Filter llank Fire Suppression System was successfully tested on 12/02/92 and was operable throughout this event.
