In accordance with Appendix A, Beaver Valley Technical Specification 3.3.3.1, " Radiation Monitoring Instrumentation," the following Special Report is submitted. This report is required when the conditions specified in Technical Specification 3.3.3.1.b are not satisfied.
On 4/10/86 at 1730 hours0.02 days <br />0.481 hours <br />0.00286 weeks <br />6.58265e-4 months <br /> RM-VS-109, one of the SPING Radiation Monitors, was declared out-of-service due to a complete unit failure.
This failure included channels 7 and 9, medium and high range noble gas channels, which monitor the ventilation system exhaust. This is nonconservative with respect to Technical Specification 3.3.3.1 and reportable per Technical Specification 3.3.3.1.b.
Investigations to determine the root cause have been performed and with the assistance of the vendor, Eberline Instruments, the problem was resolved on 4/21/86. A failure was identified on the memory board and corrective actions included the replacement of the unit software with an updated version. RM-VS-109 will be placed back in service after the successful completion of its Maintenance Surveillance Procedure. The software in the remaining SPING units will be updated as an additional corrective action. There were no safety implications to the public because alternate preplanned monitoring has been and will be available until these channels are returned to service.
