Docket No. 50-361 30-Day Report Licensee Event Report No.82-131 San Onofre Nuclear Generating Station, Unit 2 Pursuant to Section 6.9.1.13b of Appendix A, Technical Specifications to Operating License NPF-10, for San Oncfre Unit 2, this submittal provides the required 30-day written report and copy of Licensee Event Report (LER) for an occurrence involving the Auxiliary Feedwater (AFW) System.

A completed copy of LER 82-131 is enclosed.

On October 20,1982 at 2330, while in Mode 3, motor-driven AFW pump 2P-141 was declared inoperable due to failure of discharge valve 2HV4731 to stroke fully. Accordingly, Action Statement "a" of Limiting Condition for Operation (LC0) 3.7.1.2 was entered. This LC0 requires that while in Mode 1, 2 or 3, at least three independent AFW pumps and associated flow-paths shall be operable. If one train becomes inoperable, operation may continue provided the inoperable train is restored to operable status within 72 hours8.333333e-4 days <br />0.02 hours <br />1.190476e-4 weeks <br />2.7396e-5 months <br />.

Subsequent investigation revealed that valve stroke failure was caused by dirt in the oil system which prevented the valve's hydraulic pump from achieving the required actuation pressure. After replacing the oil filter and successfully testing the valve stroke time, 2P-141 was returned to operable status at 2000 on October 21, 1982.

R. H. Engelken November 18, 1982 As further corrective action to prevent recurrence, the following will be implemented:

1. Plant equipment operator's daily surveillance procedure will incorporate direction to initiate a Work Order for oil and filter changes when a high differential pressure flag is raised.
2. The plant preventive maintenance program will include a requirement to change oil and filter every six months on valve 2HV4731 and its sister valves 2HV4714, 3HV4731 and 3HV4714, the only valves of this type in Units 2 and 3.

The public health and safety were not affected by this event since any one of the two remaining AFW operable pumps (each able to provide 100%

required capacity) would have performed the necessary decay heat removal function, if required.
