Dear Mr. Keppler:

The following is in response to an inspection conducted by Mr. I. T. Yin on January 10-12, 1979 of activities at LaSalle County Units 1 and 2. Reference (a) indicated that certain activities appeared to be in noncompliance with NRC requirements. These activities are addressed in the enclosure to this letter. Upon request by this of fice on March 6, 1979, the date of response to the subject inspection reports was extended from February 7, 1979 to February 16, 1979 by Mr.

Shannon Phillips of your Staff.

Please refer any additional questions you may have on this matter to this office.

- Commonwealth Edison NRC Docket Nos. 50-373/374 ENCLOSURE COMMONWEALTH EDISON RESPONSE TO NOTICE OF VIOLATION Items of noncompliance identified in Appendix A to the NRC letter dated February 1, 1979 are responded to in the following paragraphs.

1. Contrary to 10 CFR 50, Appendix B, criterion X, the inspection program for concrete expansion anchor bolts was not considered adequate in that (1) the extent to which anchor bolts can be installed out of plumb was not specified or inspected, (2) the minimum bolt embedment length was not defined or inspected, and (3) random NRC inspection showed 40%

torque test failure on 5/8" bolts.

CORRECTIVE ACTION TAKEN AND RESULTS ACHIEVED The bolt installer (Morrison Construction Company) has established a criterion for the installation of concrete expansion anchor bolts, allowing the bolts to be installed up to 100 out of plumb. This criterion is based on recommendations from the anchor bolt manufacturer. Installation of. anchors which do not meet this requirement will be documented and submitted to the Commonwealth Edison engineering department for review and approval.

The specification for concrete expansion work, Form LS-CEA, paragraph 3.2.2, defines the minimum embedment length for wedge anchors 4 1/2 bolt diameters before tightening.

Morrison field forces have been instructed that in the f uture, anchor bolt projections shall not be cut,in order to facilitate inspection. Morrison Quality Control will perform random inspections of anchor bolt projections to insure minimum embedment requirements havc been met.

All 5/8" bolts discussed in the IE Report have been retorqued, retested and found acceptable.

Morrison Quality Control has initiated a 100% bolt torque audit of anchor bolts being installed. This program vill continue through May 31, 1979.

At the end of that period the program will be evaluated based upon the addit results.

Commonwealth Edison imC Docket Nos. 50-373/374 CORRECTIVE ACTION TO AVOID FURTHER NONCOMPLIANCE In addition to the corrective action described above, Commonwealth Edison is reviewing other on-site contractors engaged in anchor bolt installation to assure that adequate expansion anchor installation and inspection programs exist.

DATE OF M L COMPLIANCE Pull compliance is expected to be achieved on May 31, 1979 with the evaluation of the 100% audit program described above.

Contrary to 10 CFR 50, Appendix B, criterion V, the rework and replacement program for rejected concrete expansion anchor bolts was not adequate in that (1) there was no procedural provision to insure timely corrective action, and (2) there was no procedure for removing rejected bolts and installing new or replacement bolts.
