A.C. Power Sources - No. 1B Diesel Generator - Inoperable.

This report was initiated by Incident Report 83-023. ,

CONDITIONS PRIOR TO OCCURRENCE:

Mode 'l - RX Power 0 % - Unit Load 0 MWe, DESCRIPTION OF OCCURRENCE:

At 1205 hours0.0139 days <br />0.335 hours <br />0.00199 weeks <br />4.585025e-4 months <br />, January 24, 1983, during routine shutdown operation, an Equipment Operator discovered a leak in the jacket water system of No. 1B Emergency Diesel Generator. The leakage was from a mechanical joint in the system piping near Valve 1DA45B. No. 1B Emergency Diesel Generator was declared inoperable, and Technical Specification Action Statement 3.8.1.1a. was entered. Within one hour, surveillance 4 Procedures SP(0) 4.8.1.1.1a and SP(0) 4.8.1.1.2a2 were performed to demonstrate operability of the remaining A.C. sources. The surveillances were performed every eight hours until the inoperable diesel was returned to an operable status.

APPARENT CAUSE OF OCCURRENCE:

Investigation of the problem revealed that the leakage was emanating from a cracked pipe nipple. Inspection of the jacket water system piping suggested that the nipple may have cracked due to personnel standing on the piping in the course of operation or maintenance.

ANALYSIS OF OCCURRENCE:

The operability of the A.C. and D.C. power sources and associated distribution systems during operation ensures that sufficient power will be available to supply the safety related equipment for safe shutdown of the facility and mitigation of accidents. The requirements are consistent with the initial condition assumptions of the accident analyses, and are based on maintaining a single supply with an assumed loss of offsite power and single failure of the other A.C. source.

As noted, the redundant A.C. sources were operable, and therefore no risk to the health and safety of the public was involved. The occurrence constituted operation in a degraded mode permitted by a limiting condition for operation. The event is reportable in accordance with Technical Specification 6.9.1.9b.

CORRECTIVE ACTION:

As noted, surveillance of the redundant power sources was performed as required by the action statement. The cracked cipple was replaced, the leakage was stopped, and the jacket water system was-satisfactorily tested. No. 13 Emergency Diesel Generator was. declared operable ~at 0300 hours0.00347 days <br />0.0833 hours <br />4.960317e-4 weeks <br />1.1415e-4 months <br />, Januac'y- 25, 1983. and Action Statement 3.8.1.la was terminated.

The problem of personnel standing on piping was previously addressed in LER 82-053/03L.. Discussion of the potential for adverse impact on safety systems resulting from this practice has been incorporated into the station training program. Due to the human factors involved, the problem is likely to be of a persistent nature. Discussion of this incident will be added to the training program in a continuing effort

to reduce the frequency of the problem.
