l 1B Diesel - Water Jacket Leak - Inoperable ]

l CONDITIONS PRIOR TO OCCURRENCE: i Mode 3 - Rx Power 0% - Unit Load MWe.

DESCRIPTION OF OCCURRENCE:

During routine operation, an operator making rounds discovered the 1B Diesel Water Jacket was leaking via a 1/4" pipe nipple.

Action Statement 3.8.1.1 was entered at 0545 hours0.00631 days <br />0.151 hours <br />9.011243e-4 weeks <br />2.073725e-4 months <br /> on May 20, 1981.

This occurrence constituted operation in a degraded mode in accordance with Technical Specification 6.9.1.9.b.

DESIGNATION OF APPARENT CAUSE-OF OCCURRENCE:

Personnel error. This occurrence was caused by apparent use of the nipple for support by an unknown person.

ANALYSIS OF OCCURRENCE:

Technical Specification 3.8.1.la requires that with either an offsite circuit or diesel generator inoperable demonstrate the operability of the remaining A.C. sources by performing Surveillance Requirements 4.8.1.1.la and 4.8.1.1.2.a.2 within one hour and at least once per 8 hours9.259259e-5 days <br />0.00222 hours <br />1.322751e-5 weeks <br />3.044e-6 months <br />; thereafter, restore at least two offsite circuits and three diesel generators to operable status within 72 hours8.333333e-4 days <br />0.02 hours <br />1.190476e-4 weeks <br />2.7396e-5 months <br /> or be in at least hot standby within the next 6 hours6.944444e-5 days <br />0.00167 hours <br />9.920635e-6 weeks <br />2.283e-6 months <br /> and in cold shutdown within the following 30 hours3.472222e-4 days <br />0.00833 hours <br />4.960317e-5 weeks <br />1.1415e-5 months <br />.

CORRECTIVE ACTION:

The 1/4 inch nipple was replaced in kind, and Diesel Generator 1B was tested satisfactorily. Action Statement 3.8.1.1 was terminated at 1650 hours0.0191 days <br />0.458 hours <br />0.00273 weeks <br />6.27825e-4 months <br /> on May 20, 1981. Discussion of the incident will be incorporated into the station training program as an example of the possible safety significance of damage caused by personnel standing on piping and conduit.
