Auxiliary Feedwater Storage Tank (AFWST) < 200,000 Gallons CONDITIONS PRIOR TO OCCURRENCE:

Mode 2 - Rx Power 3% - Unit Load OMW DESCRIPTION OF OCCURRENCE:

During a routine surveillance of the console, it was determined the level in the AFWST was below the minimum level (200,000 gallons).

Action Statement 3.7.1.3 was entered at 1036 hours0.012 days <br />0.288 hours <br />0.00171 weeks <br />3.94198e-4 months <br /> on May 19, 1981.

This occurrence constituted operation in a degraded mode IAW Technical Specification 6.9.1.9b.

DESIGNATICN OF APPARENT CAUSE OF OCCURRENCE:

Personnel error. In the process of using the AFWST as a water supply for steam line heatup, the supply valve to the AFWST was throttled too far to compensate for the water removed.

ANALYSIS OF OCCURRENCE:

Technical Specification 3.7.1.3a requires that with the auxiliary feed storage tank inoperable, within 4 hours4.62963e-5 days <br />0.00111 hours <br />6.613757e-6 weeks <br />1.522e-6 months <br /> restore the AFST to operable status or be in hot standby within the next 12 hours1.388889e-4 days <br />0.00333 hours <br />1.984127e-5 weeks <br />4.566e-6 months <br />.

CORRECTIVE ACTION:

The AFWST was filled to > 200,000 gallons of stored water. DCR-1-EC763 and DCR-2-EC762 have been issued to provide an additional level alarm that can be acted upon prior to the AFWST level decreasing to 200,000 gallons.
