Isolation Damper 2CA A4 - Inoperable.

CONDITIONS PRIOR TO OCCURRENCE:

Mode 5 - Rx Power 0% - Unit Load 0 MWe.

DESCRIPTION OF OCCURRENCE:

On September 24, 1981, while performing Surveillance Procedure SP (O) 4. 7. 6.1, it was noticed that damper 2CAA4, in the Control Room Emergency Air Conditioning System, would not close to the correct position. At 0107 hours0.00124 days <br />0.0297 hours <br />1.76918e-4 weeks <br />4.07135e-5 months <br />, the damper was declared inoperable and Action Statement

. 3.7.6 (Modes 5 and 6)b was entered.

This occurrence constituted operation in a degraded mode in accordance with Technical Specification 6.9.1.9.b.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

The damper was sticking due to binding of the operating mechanism.

ANALYSIS OF OCCURRENCE:

Action Statement 3.7.6 (modes 5 and 6)b requires:

With both fans, the cooling coil or the filter train inoperable, or with less than 2 operable Isolation Dampers in each outside air intake duct,-suspend all operations involving core alterations or positive reactivity changes.

The damper operating mechanism was freed and tested, and damper 2CAA4 was declared operable. At 1905 hours0.022 days <br />0.529 hours <br />0.00315 weeks <br />7.248525e-4 months <br />, September 24, 1981 Action Statement 3.7.6 (modes 5 and 6)b was terminated.

Damper 2CAA4 was entered into the Maintenance Inspection Order System, and will be inspected on a scheduled basis.
