The control operator noticed the Rod Position Indicator for rod 1B4 was off scale. Action Statement 3.1.3.2.1 was entered at 2200 hours0.0255 days <br />0.611 hours <br />0.00364 weeks <br />8.371e-4 months <br /> on May 17, 1981.
This occurrence constituted operation in a degraded mode IAW Technical Specification 6.9.1.9b.
DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:
The Signal Conditioning Module in the Rod Position Indication Cabinet was out of calibration.
ANALYSIS OF OCCURRENCE:
Technical Specification 3.1.3.2.1 requires that with a maximum of one Rod Position Indicator per bank inoperable determine the position of the non-indicating rod indirectly by the moveable incore detectors at least once per 8 hours9.259259e-5 days <br />0.00222 hours <br />1.322751e-5 weeks <br />3.044e-6 months <br /> and immediately after any motion of the non-indicating rod which exceeds 24 steps in one direction since the last determination of the rod position, or reduce power to less than 50%

The unit was below 50% rated power during this entire occurrence.
