Engineered Safety Feature Actuation System (ESFAS),

Instrumentation, Safety Equipment Control (SEC) Channel 1B Malfunction.

This report was initiated by Incident Report 81-145.

CONDITIONS PRIOR TO OCCURRENCE:

Mode 2 - Rx Power 0% - Unit Load 0 MWe.

DESCRIPTION OF OCCURRENCE:

At 0454 hours0.00525 days <br />0.126 hours <br />7.506614e-4 weeks <br />1.72747e-4 months <br /> on May 20, 1981, Action Statement 3.3.2.la was entered due to failure of SEC Channel IB of the ESFAS, which was associated with the reactor trip.

This occurrence constituted operation in a degraded mode in accordance with Technical Specification 6.9.1.9.b.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

l Failure of the sample room particulate monitor produced a voltage t

transient which was of sufficient magnitude to affect the SEC i

l Channel 1B, and produce an erroneous signal.

ANALYSIS OF OCCURRENCE:

With an ESPAS instrumentation channel inoperable, the following action applies IAW Technical Specification 3.3.2.1, with the i

number of operable channels one less than the total number of j

channels:

CORRECTIVE ACTION

SEC Channel 1B chassis was replaced and maintenance procedure l

M3B was performed to determine the cause of the voltage transient; no cause was identified.

Tests were then conducted to determine the effects on the SEC system of grounding the 115VAC supply.

The tests showed that grounding could cause erroneous signals.

l The manufacturer was contacted for assistance.

After discussion it was decided that No. 1B SEC Channel could be returned to service, and that the manufacturer would investigate the problem of noise in the SEC Cabinet.

SEC Channel 1B was declared operable and returned to service, and Action Statement 3.3.2.la was terminated at 2047 hours0.0237 days <br />0.569 hours <br />0.00338 weeks <br />7.788835e-4 months <br /> on May 20, 1981.

CORRECTIVE ACTION

As a result of further testing by the Licensee and Manufacturer, Design Change Request (DCR) lEC-1386 has been implemented.

This DCR installed noise suppression circuits, which have been effective in reducing electromagnetic transients in the SEC system.
