Reactor Coolant Pump Flow Channel Inoperable CONDITIONS PRIOR TO OCCURRENCE:

Mode 1 - Rx Power 6% - Unit Load O MWe DESCRIPTION OF OCCURRENCE:

On May 20, 1981, 22 Reactor Coolant Pump Flow Channel No. 2 failed low. On May 26, 1981, the channel was still under repair with the bistable tripped. At 1704 hours0.0197 days <br />0.473 hours <br />0.00282 weeks <br />6.48372e-4 months <br /> the unit entered Mode 1.

Action Statement 3.3.1 was entered at this time.

This occurrence constituted operation in a degraded mode IAW Technical Specification 6.9.1.9.b.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

Equipment failure - Flow Transmitter 2FT425 had failed.

ANALYSIS OF OCCURRENCE:

Technical Specification 3.3.1 requires that with the number of operable channels one less than the total number of channels, startup and/or power operations may proceed until performance of the next required channel functional test, provided the inoperable channel is placed in the tripped condition within one hour.

CORRECTIVE ACTION:

The transmitter was repaired by replacing the oscillator amplifier assembly and force motor. The transmitter was calibrated and tested satisfactorily.

The Action Statement was terminated at 0209 hours0.00242 days <br />0.0581 hours <br />3.455688e-4 weeks <br />7.95245e-5 months <br /> on May 27, 1981.
