THAN 14 DAYS RESULT-IN.SPECIAL REPORT AS REQUIRED BY FIRE HAZARDS ANALYSIS A. REQUIREMENT FOR REPORT This report is required per Unit 2 Technical Specifications section 6.9.2. This section of .the Technical Specifications states that special reports for . fire protection equipment operating and surveillance requirements shall be submitted as required by the Fire Hazards Analysis (FHA) and its Appendix B requirements.

The FHA, Appendix B, section.-1.1.1, states that fire. rated-assemblies and sealing devices in fire rated assembly-penetrations separating safety related areas may be inoperable for up to 14 days. If this time limit is exceeded, a special report is required.

B. UNIT STATUS AT TIME OF THE EVENTS i

On 9/19/89, 9/25/89, 9/28/89, and 10/2/89, Unit 2 was in the refueling mode for its eighth refueling outage.

1 C. DESCRIPTION OF EVENT o This. report describes events' in which fire rated assemblies were L breached in a controlled manner fto support Unit 2 outage work and remained breached (inoperable) for more than 14 days.

On- 9/5/89 at approximately 1015 CDT, fire door IL48-C3 on the 112 L -foot elevation of the Control Building was blocked open in a l controlled manner- to run cable for a temporary transformer and temporary air compressor. The temporary transformer and air i compressor were installed- to provide station service air while the <

three, . permanent Station Service Air Compressors are being replaced-per Design Change Request 86-356. Licensed plant operations personnel declared the fire door inoperable prior to the door being blocked.open. A fire watch was established and Limiting Condition for Operation (LCO) 2-89-407 was initiated to ensure compliance with the FHA. Appendix B,- section 1.1.1, action a. As of 9/19/89 E at 1015 CDT, the fire door was still blocked open.

l In addition to fire door IL48-C3 being blocked open, the following fire rated assemblies and sealing devices were made inoperable to i support work for Design Change Request 86-356:

Fire watches were established and LCOs were initiated to ensure compliance with FHA Appendix B requirements. These fire barriers remained breached (inoperable) for more than 14 days.

D. CAUSE OF THE EVENTS L - The -fire rated assemblies and sealing devices were breached in a controlled manner to support scheduled refueling outage work

-activities, i.e., the installation of new Station Service Air i.

Compressors per Design Change Request 86-356.

d E. . ANALYSIS OF THE EVENTS Licensed plant operations personnel declared the fire rated assemblies and sealing devices inoperable and established the required fire watches to ensure compliance with FHA section 1.1.1, action a. The action statement requires that:

With one or more of the . . . required fire rated assemblies and/or sealing devices inoperable, within one hour establish a continuous fire watch on at least one side of the affected assembly (s) and/or sealing device (s) or verify the OPERABILITY of fire detectors on at least one side of the inoperable

, assembly (s) and sealing device (s) and establish an hourly fire watch patrol.

The fire rated assemblies and sealing devices' were breached- in a controlled manner using approved administrative controls. Fire  !

watches were established and all requirements of the FHA were met.

These ' steps ensured that, were a fire to occur, it would be promptly detected and . extinguished. Based on the above information, it is concluded these events did not have any adverse

'_ nuclear safety impact.

F. CORRECTIVE ACTIONS The fire rated assemblies and sealing devices were declared-inoperable -and fire watches were established per the requirements of. the FHA. - The fire barriers will be returned to an operable .

L status prior- to startup from the current Unit 2 refueling outage 1 1 .( approximately 12/20/89).
