1545 EDT DESCRIPTION OF REPORTABLE OCCURRENCE Initial Condition Unit 2,

while in cold shutdown to do maintenance on valves, replaced two IRMs that were in the core.

Nature of Occurrence While tagging the two removed IRMs with SNM

tags, it was observed that a third IRM which had been removed previously, was not in the CRD cavity of the drywell where it had been left to decay.

Immediate Corrective Action A search was conducted of the drywell, to see if it had been moved, and of radwaste, to see if the IRM was still on site.

The IRM was not located in either place.

Cause While in an earlier cold shutdown condition, a cleanup was conducted and the IRM mistakenly removed from the drywell.

It was processed with other contaninated equipment and shipped offsite for burial as it had decayed sufficiently to no longer present a radiation problem.

Supplemental Corrective Action Detector replacement procedures have been revised to onovide statements entdring that the detectors are marked as SNM when removed.

Per sonnel have been instructed so as.te prevent a recurrence.

This incident is related in that this is the first opportunity to check the Uait 2
