ENCLOSURE PLANT HATCH - UNIT 1 NRC DGCKET 50-321 OPERATING LICENSE DPR-57 SPECIAL REPORT 90-003 FIRE RATED ASSEMBLIES IN0PERABLE FOR' GREATER THAN 14 DAYS RESULTING IN SPECIAL REPORT AS RE0VIRED BY FIRE HAZARDS REPORT A. REQUIREMENT FOR REPORT This report is required by Unit 1 Technical Specifications, 4 Section 6.9.2. This section of Technical Specifications states- that special reports .for fire protection equipment operating .and surveillance requirements shall be submitted as required by the Fire -l Lnzards Analysis (FHA) and its Appendix B requirements, i The FHA, Appendix B, Section 1.2.1, states that as a minimum, the fire detection instrumentation specified for each fire detection zone shall be operable. ACTION statement b. of FHA, Appendix B, Section 1.2.1, allows for the. fire detection instrumentation to be inoperable for up to 14 days from identification of the condition. If this time is exceeded, a special report is required.

B. UNIT (S) STATUS AT TIME OF EVENTS On 4/12/90, Unit I was in -the refuel mode at an approximate power level of 0 MWt (approximately 0 percent of rated power).

C. DESCRIPTION OF EVENT On 3/29/90,a't approximately~ 1545 CST, plant personnel performing a l system walkdown discovered that twenty (20) smoke detectors located on L the working floor of the control building (common to both units) were

. not operable. The twenty detectors for panel lZ43-P400A are installed in' fire detection zone lZ43112001. Upon ' investigation of the condition, it was discovered that power and control cables to the '

l detectors had M n damaged. The damaged cables prevented a common input sig%) from detectors in zone 1Z43112001 to the plant computer.

As a consequence, the detectors in the subject zone were declared f nuperable.

The Deficiency Cards 1-90-1916 and 1-90-2368 were written to document the condition as required by the plant's administrative control procedures. Limiting Condition for Operation (LCO) 1-90-262 was initiated on 3/29/90 and an hourly fire watch was established as required by FHA, Appendix B, Section 1.2.1.

SPECIAL REPORT 90-003 FIRE RATED ASSEMBLIES IN0PERABLE FOR GREATER THAN 14 DAYS RESULTING IN SPECIAL REPORT AS RE0VIRED BY FIRE HAZARDS REPORT D. CAUSE OF EVENT The cause of the event was determined to be personnel error by persons unknown. The affected cables were apparently damaged during work activities involving replacement of the three Unit 1 Station Service Air Compressors under Design Change Request (DCR)86-355. The damaged cables were routed through the 1.wediate area involved in the DCR work.

E. ANALYSIS OF EVENT In this event, licensed plant operations personnel declared the fire detection instrumentation inoperable and established the required fire _

watches to- ensure compliance with FHA, Appendix B, Section 1.2.1, Action a, The actions statement requires that:

Within 1 hour1.157407e-5 days <br />2.777778e-4 hours <br />1.653439e-6 weeks <br />3.805e-7 months <br />, establish a fire watch patrol to inspect the zone with the inoperable instrument (s) at least once per hour.

This action ensured that if a fire had occurred, it would have been promptly- detected and extinguished. Additionally, linear heat detectors mounted in the affected fire zone were operable at the time of the event and would have performed as designed in the unlikely event of a fire.

Based on the shove information, it is concluded that this event had no adverse 'impac.t on. nuclear safety. The above analysis is applicable to all power levels.

F. CORRECTIVE ACTIONS The corrective actions for this event include:

1. Limiting Conditions for Operation (LCO) was initiated.
2. An hourly fire watch patrol was established for the affected area. -
3. The affected -fire detection instrumentation and cables were repaired, functionally tested and then returned to operable status on 4/25/90. The corrective actions were not completed within 14 days from discovery due to the workscope associated with the 000486.

4. Since.the personnel involved in this event could not be identified, no counselii.9 of personnel was performed. This appears to be an isolated case, and therefore, no further action is required at this
