CAUSE/ CORRECTIVE ACTION / COMMENTS:

  • cables. The cables were exposed to excessive heat resulting in degradation of the electricalinsulation.

The conduits and power cables to the "C" and "K" SRVs were replaced. The cables for the remaining SRVs were checked and none were found damaged. The drawings used to inspect the installation are being revised to schematically show proper routing of the power cables and add a caution that the conduit must not contact uninsulated metal or be covered by insulation.

l NARRATIVE REPORT Unit 2 began the month of February in a maintenance outage to repair electrical grounds on the "C" and "K" safety relief valves. Shift tied the main generator to the grid on 2/3/99 and began ascension to 2708 CMWT.

The power ascension was stopped at approximately 95% of rated thermal on 2/5/99 due to thermal limits.

Shift reduced power to approximately 75% of rated thermal later that day to perform a Rod Pattern Adjustment. The unit attained 2708 CMWT on 2/6/99. Shift continued to maintain operation at approximately 2708 CMWT for the remainder of the month.
