I ENCLOSURE 1 )

i NOTICE OF VIOLATION I I

Virginia Electric and Power Company Docket Nos. 50-338 and 50-339 North Anna Units 1 and 2 License Nos. NPF-4 and NPF-7 .

1 I

During the Nuclear Regulatory Commission (NRC) inspection conducted on j May 13 - June 16,1987, violations of NRC requirements were identified. The 1 violations involved a failure to comply with Technical Specification (TS) j 3.6.1.1 and failure to follow refueling procedures. In accordance with with the

" General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1787), the violations are listed below: )

A. Technical Specification 3.6.1.1 requires primary containment integrity to be maintained in modes 1, 2, 3 and 4. Containment integrity is maintained by verifying that all penetrations not capable of being closed by operable containment automatic isolation valves and required to be closed during accident conditions are closed by valves, blind flanges, or deactivated automatic valves secured in their positions, except as provided in Table 3.6-1 of Specification 3.6.3.1.

Contrary to the above on numerous occasions during the past two years the performance of TS 4.4.6.2.a, surveillance requirement for the hydrogen recombiner, was conducted with the associated unit in modes 1, 2, 3 or 4.

The performance of this surveillance requires manually operated contain-ment isolation valves listed in TS table 3.6-1 to be open for greater than 1 one hour violating the containment integrity TS 3.6.1.1. The last perfor-mance of this surveillance was March 6,1987 for Unit 1 which was operating at 100% and May 21, 1987 for Unit 2 which was operating at 100%.

This is a Severity Level IV violation (Supplement 1) and applied to both units.

B. Technical S p .fication 6.8.1.b requires written procedures to be established, implemented and maintained covering refueling operations.

Contrary to the above on May 16, 1987 the refueling building bridge operator violated the licensee's refueling procedures and policies by operating the bridge in the lateral direction with an attached saent fuel assembly still inserted in the fuel storage rack and operating tie bridge crane with only one operator when licensee policies require two. This failure to follow procedure resulted in the potential for damage to a spent fuel assembly. The assembly was removed from the rack, inspected and determined not to be damaged.

This is a Severity Level IV violation (Supplement 1) and applies only to Unit 1.
