Special Report on 9.adiation Monitor RM-063 Inoperability Gentlemen:

Fort Calhoun Station Technical Specification Table 2-10, footnote (a) states that:

"With the number of OPERABLE channels less than required by the minimum channels operable requirements, initiate the pre-planned alternate methods of monitoring the appropriate parameter (s) within 72 hours8.333333e-4 days <br />0.02 hours <br />1.190476e-4 weeks <br />2.7396e-5 months <br />, and

1. either restore the inoperable channel (s) to operable status within 7 days of the event, or l
2. prepare and submit a special report to the Commission pursuant to Specification 5.9.3 within 14 days following the event outlining the action taken, the cause of the inoperability, and the plans and schedules for restoring the system to operable status."

This letter is to inform you of the inoperability of the Post Accident Wide Range Noble Gas Stack Monitor, RM-063L, M and H, and the plans for returning it to service.

RM-063 was taken out of service for calibration of flow controllers per CP-063F on May 5, 1987. The plant was shutdown for refueling at this time. CP-063F was completed and approved on July 14, 1987. RM-063L electronic and secondary calibration per CP-063L was begun on May 30, 1987, and completed on June 6, 1987. The secondary calibration failed, requiring performance of the primary calibration. CP-063L was completed and approved on July 20, 1987.

RM-063M was taken out of service on May 19, 1987, for calibration per CP-063M.

CP-063M was completed and approved on July 20, 1987. RM-063H was taken out of service for calibration per CP-063H on May 22, 1987, and has not been returned to service as of August 7, 1987. Replacement parts nece:sary to return the monitor to service are on order.

  • Document Control Desk Page Two LIC-87-563 During this period, the refueling outage ended and the plant reached Mode 3 on June 2, 1987. At this time, Technical Specification 2.21 and Table 2-10 became applicable, requiring the monitors to be returned to service by June 9,1987, or the submission of a special report by June 16, 1987.

The schedule for returning RM-063 to operation is as follows:

1. RM-063L - in service on July 20, 1.w7.
2. RM-063M - in service on July 20, 1987.
3. RM-063H - primary calibration and procedure to be completed by September 30, 1987.

It is noted that the requirements of Technical Specification 2.21 and Table 2-10 were not met. This is a violation and was observed by NRC Representatives during an inspection of the Radiation Protection Program, June 22-26, 1987, during the exit interview (see Inspection Report 87-16). Licensee Event Report 87-023 was prepared to document the events.

This letter is submitted to meet the requirements of Table 2-10. It is not intended to address the question of a Technical Specification violation or the corrective actions associated with the issue. These will be addressed in OPPD's response to Inspection Report 87-16. If there are any further questions, do not hesitate to contact me.
