NOTICE OF VIOLATION

'

Virginia Electric and Power Company Docket No. 50-338 North Anna Unit 1 License No. NPF-4 The following violations were identified during an inspection conducted on September 2 - October 6,1985.- The Severity Levels were assigned in accordance with the NRC Enforcement Policy (10 CFR Part 2, Appendix C).

Technical Specification (TS) 6.8.1 requires that written procedures be established, implemented and maintained covering the areas recommended in Appendix A of Regulatory Guide 1.33, Revision 2, February 1978, which includes both the areas of equipment control (tagging and locking) and operation of the Auxiliary Feedwater System during both normal and abnormal condition Contrary to the above, the requirements of the TS were not fully implemented in that: Tagging requirements in Station Administrative Procedure (ADM) 14.0, Tagging of Systat. and/or Components (March 31, 1985), were not followed during the repair of motor operated valve MOV 1588 (Work Order 5900029683). Abnormal Operating Procedure (AP) 1-AP-22.3, Placing 1-FW-P-3A and/or

3B In ' Service To Feed The Steam Generators Via The MOV Header (November 29, 1979), was inadequate in that, the procedure did not require monitoring pump inlet water temperatur System design requirements of 120F were exceeded during pump operation on September 15, 198 This is a Severity Level IV violation (Supplement I) and applies only to Unit Pursuant to 10 CFR 2.201, you are required to submit to this office within 30 days of the date of this Notice, a written statement or explanation in reply, including: (1) admission or denial of the alleged violations; (2) the reasons for the violations if admitted; (3) the corrective steps which have been taken and the results achieved; (4) corrective steps which will be taken'to avoid further violations; and (5) the date when full compliance will be achieve ' Security or - safeguards information should be submitted as an enclosure to facilitate withholding it from public disclosure as required by 10 CFR 2.790(d)
