Dear Mr. Pandey:

As we previously discussed, enclosed is our application for a general storm water permit for construction activity at North Anna Power Station.

The construction activity is associated with the installation of a spent nuclear fuel storage facility. Specific activities include clerring the site, constructing an engineered fill, and installing a two-foot thick concrete pad. The total disturbed area is approximately 11 acres and there are two discharge points to Lake Anna via the Waste Heat Treatment Facility..

Included with the application are the completed Local Government Ordinance Form, the pink copy of the Permit Application Fee Form, and a copy of our $80.00 check for the application.

' Fe'es are'ako required for registration for coverage under General Permits except for the general permits for sewage treatment systems with discisarges of 1,000 gallons per day (GPD) or less and for Corrective Action Plans for leaking underground storage tanks. Except for VWP permits, fees must be paid when applications for permit issuance, reissuance or modification' are submitted.
