' U.S. Nuclear Regulatory Commission Document Control Desk December 5, 1988 ATTACHMENT Ouke Power Company McGuire Nuclear Station Diesel Generator 1B Special Report On November 5,1988 at 0428, Diesel Generator (DG) IB experienced an invalid test failure. Operations was in the process of performing the DG operability periodic test,(PT/1/A/4350/028) when the test failure occurred. The DG did not trip but the DG breaker did trip. An evaluation performed by Operations revealed that the OG breaner tripped due to improper adjustment of the voltage regulator during the operability test; therefore, the test failure has been attributed to personnel error. The DG was shutdown per procedure and the operability test was performed again. The results of this test revealed that no problems existed with the DG or DG controls, i

4 When the voltage regulator was improperly adjusted during the test run, the DG breaker tripped because the power factor and VARS were excessive; therefore, the

OG breaker performed properly for the conditions that existed during the test.

4 Operations management will cover this incident with a representative from each Operations shift in en upcoming Shift Supervisors meeting.
