Diesel Generator Special Report Due To A Valid Failure Of Diesel Generator 2A on June 12, 1995, As Documented On Problem Investigation Process (PIP) 2-M95-1104.

July 12,1995 On June 12,1995, Diesel Generator (DG) 2A was being run following maintenance using routine surveillance test procedure PT/2/A/4350/02A, Diesel Generator 2A Operability Test. The DG was i
approximately 20 minutes into a one hour operability run (start 1004). At approximately 1600 the turbocharger failed, as detected by excessive noise and vibration. The engine was subsequently secured. Unit 2 was in Mode 1, Power Operation, at the time of the failure.
This was the first Valid Failure in the last 20 Valid Tests and the fourth Valid Failure in the last 100 Valid Tests of DG 2A. On a Unit basis, this was the third Valid Failure in the last 100 Valid Tests of DGs 2A and 2B. Surveillance testing remains monthly per the requirements of Technical Specification 4.8-1 (Diesel Generator Test Schedule).

Upon inspection, damage noted included a single ejected compressor impeller blade, a damaged compressor end bearing, and a damaged turbine end bearing. Prior to completing the final root cause analysis for DG 2A's failure, the DG 2B turbocharger suffered an identical failure during operation. A preliminary engineering analysis following this failure revealed a recent design change that added air assist nozzles which can cause individual impeller blades to reach resonance under the specified testing conditions.
Corrective actions as a result of the DG 2A turbocharger failure included the replacement of damaged components and retuming DG 2A to an operable status.
Additional information regarding this failure, the DG 2B turbocharger failure, and the impact on the Unit 1 DG turbochargers will be included in Licensee Event Report 369/95-03.
