Loose Parts Monitor Special Report On June 2, 1990, Operations personnel discovered that Channel'7 of the j

t Vibration and Loose Parts Monitor (VLPM) system had a low noise level. The likely cause'of.the channel. failure.is a bad or damaged sensor cable.

was not possible to determine the definite cause of the failure because-these devices are located inside containment = preventing inspection during power operation.-

1 The VLPH is manufactured by Rockwell International.- The VLPM has had a l

history of not providing reliable service.

Cables have had.a history.of' j

being:very susceptible to damage causing system failure. This system is

currently scheduled to be1 replaced by a more reliable system during the next j

scheduled Unit i refueling outage in 1991.

In the. event this modification is noc implemented. repairs'will be made per work request no. 141364.

At:this.. time, no compensatory measures are planned or required.
