I DUKE POWER COMPANY McGUIRE NUCLEAR STATION DIESEL GENERATOR 1A SPECIAL REPORT On June 27, 1988 at 0153 Diesel Generator (D/G) 1A was started for an operability test (start attempt No. 687) per the Operability Test procedure, and terminated 33 minutes later.
The time required to reach 4160 Volts was 11.004 seconds as recorded by the events recorder.
McGuire Technical Specifications require the D/G to start and achieve 4160 Volts within 11 seconds.
This failure was the fourth valid failure out of 100 valid start attempts for the Unit 1 D/G sets.
At 0359, D/G 1A was started for trouble shooting (start attempt No. 688) to determine the cause of the failure associated with start attempt No. 687. The start time was 11.056 seconds. Another start at 0505 (start attempt No. 689) resulted in an 11.045 second start.
At 0604, start attempt No. 690 required 11.173 seconds.
From the trouble shooting it was determined that the drain valve on the starting air filter housing on the right bank side of the diesel engine was missing, causing air to leak during the time in which starting air was being supplied to the engine.
A work request was written to replace the filter housing.
After the replacement of the filter housing at 1724, D/G 1A was started for an operability test (start attempt No. 691) with a start time of 10.713 seconds.
After an 82 minute run the D/G was declared operable.
Instrumentation and Electrical personnel are revising the appropriate preventative maintenance tasks to check the condition of the starting air oil misters and filter drain valves every 12 months.
Throughout this incident Unit I was in Mode 1, Power Operation.
