On February 25, 1989 Operations personnel discovered that channel 8 of the l
Vibration and Loose Parts Monitoring System (VLPM) had a low noise level. A J
work request was written to investigate and repair the channel; however, it has not been possible to determine the actual cause of the failure due to the numbers of possible failures (e.g. bad cable, failed accelerometer, failed pre-amp, etc.) and due to the location of the components within the plant (all components are currently inaccessible due to the work associated with a tube leak on 'D' steam generator). The investigation and repair of the VLPM are scheduled to be completed during the current Unit 1 tube leak outage. Noise levels in this channel are lower (quieter) than normal and lower than surrounding channels.
If a loose parts condition did exist, a change in the noise level of this channel would be observed.
The VLPM is manufactured by Rockwell International and has had a history of not providing reliable service. Duke Design Engineering has conducted an investigation of the system which identified several design flaws and is presently pursuing a replacement for the existing VLPM.
The modification has been scheduled to be implemented for Unit 1 in 1991.
This item is not reportable to the Nuclear Plant Reliability Data System.
